Description

Position Summary:

Provides consultative services to medical, nursing and pharmacy staff. Performs clinical and professional pharmaceutical work in the compounding and dispensing of drugs and IV supplies according to professional standards and practices under the direction of the Pharmacy Director. Assumes supervisory pharmacist responsibilities in absence of Pharmacy Director.

Position Designation:

The position of Pharmacist is identified as department safety-sensitive.

Department safety-sensitive positions mean employment positions which may in the normal course of business:

  1. Require the employee to operate the company vehicles or heavy equipment, or
  2. Involves duties which, if performed with inattentiveness, errors in judgment, or diminished coordination, dexterity, or composure, may result in mistakes that could present a real and/or imminent threat to the personal health and safety of the employee, co-worker, and/or the public.

Delegation of Authority:

As a Pharmacist, you are delegated the administrative authority, responsibility and accountability necessary for carrying out your assigned duties.

Duties and Responsibilities:

1. Conducts Drug Utilization Reviews and prepares Drug Review Criteria for review by QI, P & T, infection control, medical staff and/or other committees as required/requested.

2. Consults with and advises medical staff personnel regarding utilization of drugs and pharmaceuticals as they relate to patient care.

3. Keeps abreast of latest developments concerning new drugs or developments regarding drug usage.

4. Advises staff/caregivers, using good clinical judgment, on the correct use of various medications including dosages, side effects, composition, food/drug interactions, stability and other relevant information.

5. Assists with department QI program including preparing reports, collection, collation and analyzing of data. Able to identify problems, issues or trends related to any function of department or patient care. Able to communicate information to others to generate safer practices and provide quality patient care.

6. Monitors drug therapy for clinical appropriateness (disease status conflicts, adverse drug reactions, incompatibility issues, appropriateness for age, weight, diagnosis, duplicate therapy issues, etc.) and demonstrates appropriate interventions.

7. Monitors for drugs which require PAR’s or are non reimbursable by Nevada Medicaid, potential drug interactions and allergies. Makes reasonable suggestions or interventions for alternate therapy.

8. Prepares clear and concise written clinical briefs for use by medical staff and pharmacy.

9. Ensures delivery of quality pharmacy services for inpatients, outpatients and employees.

10. Identifies and follows Policies and procedures, rules and regulations, protocols, and standards of care and notifies supervisor when breeches occur or when there is potential for errors.

11. Checks stock regularly to identify and reorder outdated stock and ensures that stock is maintained in accordance with manufacturer requirements. Identifies pricing or product changes and demonstrates appropriate follow-up.

12. Supervises pharmacy technicians in their daily duties.

13. Ensures secure storage of narcotics and other controlled substances. Ensures maintenance of perpetual inventory of controlled substances. Notifies supervisor of any irregularity.

14. Assists with maintenance of perpetual narcotic inventory and other records such as inventory, prescriptions, tax free alcohol, and quality assurance.

15. Assists in preparation of various required reports by compiling, recording and filing needed information.

16. Reviews and assists pharmacy tech to prepare the pharmacy charges for all patients. Identifies charting discrepancies and errors and documents and forwards the information for appropriate correction.

17. Assists and supervises pharmacy techs to check incoming shipments and arranges for their storage, inventory stock levels, forwards new items or price changes to the supervisor, etc.

18. Assists in supervision and training of new pharmacists and pharmacy techs.

19. Actively participates in the review of policies, procedures and protocols on an annual basis. Assists with the development of and revisions to assure they meet current practice and standards.

20. Assists with or participates in departmental projects as requested/required.

21. Maintains knowledge of regulatory standards and standards of care as applied to the pharmacy.

22. Performs all job functions according to Federal, State and local regulations and according to standards of care.

23. Demonstrates technical competency and safety through the ability to select the best pharmaceutical methods for all necessary procedures, by maintaining certifications of any specialized area of training and by attending training and inservices as available.

24. Maintains harmonious relationship and open line of communication within the pharmacy department, other departments, staff, physicians and other support personnel providing support and assistance to ensure quality patient care.

25. Maintains a positive and professional atmosphere with patients, their families, the public, other agencies, etc. demonstrating interpersonal skills necessary to interact effectively.

26. Participates in rotation of stand-by call and telephone consultations responding according to department policies and procedures and needs.

27. Responsible for all dispensing functions and issues of the department while on duty or on-call.

28 Maintains confidentiality of all patient/resident/staff information.

29. Performs other related duties as assigned or requested.

Position Qualifications:

Preferred Minimum Education:

  • Bachelor’s Degree in Pharmacy from an accredited school of pharmacy with internship emphasis on hospital pharmacy
  • One year ASHP residency in a hospital pharmacy or equivalent level of training and experience

Required Licensure/Certification:

  • Valid Nevada Pharmacist License

Personal Qualifications:

  • Able to read, write and speak the English language in an understandable manner
  • Must have good oral and written communication skills
  • Two years previous experience as a hospital pharmacist preferred
  • Ability to deal tactfully with personnel, patients, family members, visitors, and general public
  • Possess ability and willingness to work harmoniously with professional and non-professional personnel
  • Must be able to work weekends, holidays and occasional extended hours
  • Must be able to be on stand-by call and respond according to department policies and procedures
  • Knowledge of computer and typing skills
  • Must be self-motivated, have the ability to make independent decisions when circumstances warrant, and have problem solving skills
