HRCHC is Hiring a Nurse Practitioner - FNP Near Portsmouth, VA

Hampton Roads Community Health Center, a federally qualified health center, is currently recruiting a Nurse Practitioner to join the HRCHC team in Portsmouth, VA!

Key Duties and Responsibilities:

  • Provides primary care services of an acute, chronic and preventive nature to patients of all age groups as guided by organizational protocols and license to practice.
  • Plans diagnostic studies for each patient as necessary; evaluates all diagnostic results; consults with supervising physician as appropriate;
  • Diagnosis and treatment of common acute and chronic illness, including minor trauma, episodic problems, common chronic disorders, as well as counseling and education with regard to such problems, performance of minor surgical procedures and initiation of life-saving procedures in emergencies.
  • Writes prescriptions for required medication and/or treatment to meet the management plan, as allowed by state regulation and/or clinical protocols.
  • Refers complex and high priority cases to the physician on duty with regard to complicated diagnostic problems, serious illness, complicated therapeutic problems, reevaluation of chronic conditions, etc.
  • Completes Electronic Medical Records according to practice standards and closes charts within the appropriate time frame.
  • Serves as preceptor for medical and nursing professionals and other students as reasonably assigned
  • Writes referrals and works with other staff to effectively communicate information to patients and their families. Utilizes medication assistance services as appropriate.
  • Assists in the development of protocols and procedures for preventive and/or special services.
  • Participates in community outreach efforts, including health fairs, neighborhood events, and other public forums for delivering screening services, education, and referrals. Collaborates with other staff to maximize patients access to full range of health and human services.

Required Education and Experience:

  • License to practice as Nurse Practitioner (Family Nurse Practitioner certification preferred) in the Commonwealth of Virginia, with prescriptive authority.
  • CPR (Basic Life Support) Certified.
  • Must have the ability to effectively communicate with physicians, patients, and staff, with strong customer service orientation.
  • Team-oriented, with ability to share authority and responsibilities for various clinical and administrative functions. Interacts effectively with patients from diverse backgrounds.
  • Demonstrated enthusiasm for wellness-based practice and incorporation of faith/beliefs in patient s health practices.
