Hope Alive Care Services is approved by the state of Maryland to provide Psychiatric Rehabilitation Program for minor and adults . This is a contractual position. The PRP Coordinator/Counselor advocates for clients and assists in their independence and integration within their community.

Job Expectations:

• Provide a minimum of 3 visits per month for minors clients and 6 times per month for adult clients..
• Complete client progress notes within a 24 to 48-hour timeframe according to the goals on the Individual Rehabilitation Plan.
• Ensure that the treatment plans are done in a timely manner.
• Provide collaboration with mental health therapist monthly.
• Work cooperatively with family and other involved professionals to coordinate services with other agencies and programs in the best interest of the client
• Must have valid driver’s license and be fully insured.

Requirements: Interested candidates should have a minimum of an A.A. Degree and/or 30 college credits in a health-related field or at least one year of relevant work experience. Candidate must be computer literate and demonstrate strong organization, time management and writing skills.
