Hispanic Federation is Hiring a Health Programs Coordinator Near New York, NY

Who We Seek

Hispanic Federation is seeking an engaged Health Program Coordinator to execute community engagement and lead workshops within network and partner agencies. This position will work on the Vida Saludable program, a pre-diabetes program with a standard curriculum focused on healthy lifestyles. The coordinator will oversee the program logistics, provide support to participants, and recruit and engage partner agencies in Atlanta, GA; Dallas, TX; and Houston, TX.

The coordinator will support program administration and manage logistics for events in community spaces. The successful candidate will have experience in program management, expense tracking and budget management, and an interest in supporting healthy lifestyles in Latino communities.

The position is a hybrid role which will require a minimum of 10 days onsite or in the field per month. Candidates should be in New York City and must be able to report to the HF Headquarters.

Responsibilities may include:

  • Assists in monitoring program effectiveness and oversee administration; demonstrate continuous efforts to improve support operations and systems, to streamline work processes and to increase the effectiveness and efficiency of the program.
  • Prepare reports for review, manage data collection and analysis of program progress by managing program logistics and deliverables.
  • Cultivate and manage partnerships in the program areas identified.
  • Support lifestyle coach for diabetes program by periodically serving as a coach.
  • Assists in the planning, coordination and implementation of curriculum and class participation which includes:
    • Encourage group participation and interaction during classes.
    • Create a motivating and noncompetitive environment.
    • Foster relationships with and between participants.
    • Prepare before each class, reviewing participants' records and making reminder calls.
    • Be accessible to participants before and after sessions.
  • Support goal setting on a weekly basis.
    • Record session data for each participant (attendance, body weight, physical activity minutes, etc.).
  • Execute follow up with participants outside of class if they are unable to attend.
  • Attend meetings/trainings as directed and/or required.
  • Comply with all applicable laws and regulations, including those governing privacy and data security.

Eligibility and Qualifications

  • Bachelor’s degree required or equivalent education and experience, a minimum of3-5 years of experience working as a Community Health Worker with adults from communities that have a disproportionate burden of poverty and chronic disease.
  • Specialized training as a CDC Diabetes Prevention Program lifestyle coach is desired. Willingness to train as a Lifestyle Coach within the first two months of hire (covered by the program) if not already certified, is required.
  • Past experience leading groups and group facilitation is preferred, but not required.
  • A passion for teaching people about healthy lifestyles and motivating participants to learn.
  • Knowledge, and experience in basic health education, and nutrition and/or exercise program implementation.
  • Knowledge of the principles of behavior change, including motivational interviewing techniques.
  • Skilled in verbal and written communication, well organized, computer literate, data collection, and capable of managing a variety of activities.
  • Ability to build strong relationships with individuals and foster community within a group.
  • Bilingual in Spanish is a plus.
  • Ability to guide behavior change efforts in others without prescribing personal actions fostering self-confidence in participants.
  • Ability to communicate empathy for participants, especially during challenging times in adopting and sustaining healthy lifestyle changes.
  • Must be willing and able to travel as part of the program deliverables.

Compensation and Benefits:

Salary is commensurate with experience and in the range of $45,000-50,000 benefits package includes paid vacation, paid medical, dental, and vision insurance, and a company match for retirement planning after six (6) months.
