Job Summary

The Care Manager assumes responsibility and accountability forassisting in the planning, organization, supervision, evaluation, delivery, andreporting of all care and nursing services in the assisted living and memorycare facilities. The Care Manager will be assigned a caseload in the facilitiesand shall assist the Community Manager in ensuring the direct care team iscompliant with federal, state, and local regulations and implementation of thecommunity’s programs and policies. Follows all mandated disease controlpolicies. Adheres to the budget policies and participates in census building. Help strategize with new processes to adhere to best practicestandards and quality measures. Implements andfollows through of new policies, procedures, programs, and processes asdirected by the Director of Clinical Services.

Major Responsibilities

1. Ableto perform and complete proficiently in all clinical competency areas includingbut not limited to: training, charting/documentation, direct patient care, medicationadministration/management, patient rights, infection control, safety, andsanitation.

2. Collaborate with physicians, pharmacists, and other clinical providersto coordinate care and services for the residents to ensure residents’treatment requirements are met.

3. Manage caseload to complete Admission Service Care Plans bothdocumentation and 1:1 resident evaluation to promote continuity of care andquality health outcomes.

4. Complete/Reportthe results of the Admission Tool (LOCAT) within 48-72 hours of request. Collaborate with Sales partners to determine communitycapability and assess potential residents’ appropriateness for move- in.

5. Completeresident admission clinical assessments evaluating for pain, skin, and health.

6. Overseeresident charts for accuracy including chart auditing. Obtain MedicationRecords, TB test, Durable Power of Attorney, and end of life choices documentsare placed in the chart.

7.CompleteLevel of Care (LOC) evaluations as scheduled for appropriate staffing forpatent care.

8. UtilizeClinical Rounding Tool as the structured,and goal-oriented rounding strategy aimed at improving rounding efficiencywhile maintaining the foundational aspects of patient care and clinicaleducation critical to resident care delivery and Interdisciplinary Team collaboration.

9. Completethe reporting requirements when there is a change in resident condition to notifythe Interdisciplinary Team and Supervisors and follow the Family CommunicationPolicy.

10. Assist with admissions, discharges, and residents’ transitions throughoutthe continuum .
Practiceinfection control policies and procedures.

11.MonitorMedication effectiveness, adverse effects, and report these to Physician, MedTechs and Care Assistants.
Coordinateend of month Medication Administration Record change over.

12. Followthe medication disposal policy, pharmacy protocol for medication reviews, attendtraining.

    Required Certifications, Licenses, and Registrations

    1. Licensed RN or LPN in the State of Michigan required for NurseCare Manager.

    2. Current CPR certification.

    EssentialPhysical Functions

    1. Ability to lift 25 pounds.

    2. Ability to tolerate walking.

    Experience

    1. Clinical nursing experience of not less than 2 years.

    2. Management experience preferred. 

    3. A knowledge and interest in the geriatric nursing field.
