Van Buren Community Mental Health is Hiring a RN Care Coordinator Near Paw Paw, MI
RN Nurse Coordinator needed to join our team in Paw Paw, MI, just 20 minutes west of Kalamazoo. The RN Coordinator will identify and link customers of behavioral health services with medical resources in the area, make appropriate referrals, coordinate care, and provide follow up. Duties include identifying gaps in care, helping customers meet treatment and life goals, address barriers to receiving services, and acting as education source to customers, partners and staff. Provides nursing services in Medication Assisted Treatmentclinic. Will also receive CPR/First Aid Instructor certification to provide training to staff.
Candidates must possess licensure as a Registered Nurse in Michigan and demonstrated knowledge of nursing as it relates to working with individuals with mental illness and substance use disorders. Must be able to use an Electronic Medical Record for customer documentation.
Position is full-time (37.5 hours per week), M-F, 8:30am-5:00pm. Salary range is $48,601-$65,316 per year plus an excellent benefit package including an affordable, top-notch health/dental/vision plan, deferred compensation retirement plan with employer matching, employer-provided life & disability insurance, 12 paid holidays each year, and generous paid time off to promote work/life balance.
