Position Overview:

The LVN Case Manager plays a vital role in our healthcare team, providing comprehensive care coordination and support to patients in service. This position requires a skilled and compassionate individual with strong clinical knowledge and excellent communication skills. The LVN Case Manager will collaborate closely with patients, families, and interdisciplinary team members to ensure the delivery of high-quality, patient-centered care.

Key Responsibilities:

• Patient Assessment and Care Planning:
• Conduct thorough assessments of patients' medical history, current health status, and care needs in preparation for infusion therapy.
• Assist in the development of individualized care plans in collaboration with registered nurses or healthcare providers, focusing on promoting optimal health outcomes and patient well-being.

Treatment Monitoring and Follow-Up:

• Monitor patient response to infusion therapy, assessing for any adverse reactions or treatment complications.
• Maintain regular communication with patients to ensure adherence to treatment plans and address any concerns or questions they may have.
• Collaborate with healthcare providers to adjust treatment plans as needed based on patient response and clinical indicators.

Patient Education and Support:

• Provide education to patients and their families/caregivers regarding infusion therapy procedures, potential side effects, and self-care strategies.
• Offer emotional support and counseling to patients and families, helping them cope with the challenges associates with chronic illnesses and infusion therapy
• Advocate for patients' rights and preferences, ensuring that their voices are heard and respected throughout the care process.

Documentation and Reporting

• Maintain accurate and up-to-date documentation of all patient interactions, assessments, and interventions in accordance with regulatory standards and organizational policies.
• Generate reports and communicate patient progress to registered nurses, healthcare providers, and other members of the healthcare team as needed.

Quality Improvement and Compliance:

• Participate in quality improvement initiatives aimed at enhancing the delivery of care within the infusion center.
• Ensure compliance with all relevant regulatory standards, policies, and procedures governing infusion therapy and patient care.

Qualifications:

• Graduation from an accredited LVN program and current licensure as a Licensed Vocational Nurse (LVN) in the state of practice.
• Minimum of 6 years of clinical experience in a healthcare setting, preferably in case management or care coordination.
• Strong assessment, organizational, and time management skills.
• Excellent communication and interpersonal skills, with the ability to effectively interact with patients, families, and healthcare team members.
• Proficiency in electronic health record (EHR) systems and other relevant software applications.
• Commitment to providing patient-centered care and promoting positive health outcomes.
