Healthcare For the Homeless Inc is Hiring a Case Manager - Community Site (Part - time) Near Baltimore, MD

 
 

Overview

As a critical member of an interdisciplinary team, the Case Manager provides case management services to adult, pediatric and geriatric clients who are receiving medical or behavioral health services. The Case Manager is pivotal in addressing social determinants of health by assessing the client’s needs and connecting them to appropriate services.

Key Role Responsibilities

  • Provides case management assessments, treatment planning and interventions. Engages clients and builds relationships by greeting clients with a warm introduction, communicating with empathy and compassion, and working on goals identified by the client.
  • Collaborates with other providers in an integrated, multidisciplinary care team, attending interdisciplinary meetings as required. Engages other team members in problem solving regarding client care, and links clients to ongoing medical, behavioral health, and supportive services.
  • Assertively and systematically identifies clients who are eligible for financial, housing and other benefits. Assists clients in applying for financial, housing and other benefits, including completing Coordinated Access applications, other housing applications, disability benefit applications, or employment related services. Identifies and supports the client’s use of appropriate external resources. 
  • Provides referrals to crisis intervention services such as emergency shelter and food assistance.
  • Completes documentation of services in a thorough manner in accordance with required timeframes within the client’s electronic health record. Documentation includes but is not limited to: initial assessments, treatment plans, progress notes, and discharge summaries.
  • Effectively supports the client’s case management plan by escorting the client to appointments and visiting clients in their home or in the hospital as needed.

Key Agency Responsibilities 

In addition to role responsibilities, each staff member of Health Care for the Homeless has the following responsibilities as a part of their employment:

  • Models and reinforces the Health Care for the Homeless “core values” of dignity, authenticity, hope, justice, passion and balance
  • Actively participates in performance improvement activities and actively participates in advocacy activities that support the mission of Health Care for the Homeless
  • Performs other duties on an as-needed basis
  • Protects our client’s personal health information by maintaining compliance with HIPAA and other relevant Health Care related IT security regulations

Knowledge, Experience, and Skills 

Formal Education and Training

  • Bachelor’s degree in human services field preferred: five years of relevant work experience may be substituted 
  • Bilingual Spanish-English (Conversational Spanish) highly preferred
  • Personal vehicle and valid Maryland driver’s license required

Experience 

  • Two years of case management experience preferred
  • One year of experience in a health care setting or community-based organization preferred
  • Experience working with individuals from diverse backgrounds required

Skills 

  • Knowledge of assessment and treatment planning
  • Able to describe how they support and uplift BIPOC and TGNC individuals and communities in their work
  • Willingness to integrate principles into practice such as Harm Reduction, Motivational Interviewing and Housing First
  • Familiar with Baltimore City, community resources, and locations of relevant service providers
  • Excellent verbal and written communication skills
  • Strong organizational and time management skills
  • Able to cope with interruptions and be a team player
  • Flexible approach, working with several cross-disciplinary teams in a collaborative style
  • Approaches change with a positive, open-minded attitude
  • Able to work with clients who have significant mental health challenges, manage crisis situations and assess for safety
  • Able to work with ill, disabled, emotionally upset, and sometimes hostile clients
