Through its core values of innovation, engagement, and health equity, HCPH strives to bring meaningful solutions to public health issues while keeping Harris County healthy and vibrant.Position Overview:The ACCESS Eligibility Specialist is a front-line public health worker who is a trusted member of and has an unusually close understanding of the communities served by ACCESS Harris. Under the direction of the ACCESS Harris Cohort manager, the ACCESS Eligibility Specialist is responsible for completing assessments, to meet with cohort participants, develop partnerships, scheduling appointments, determining program- participant eligibility and preparing in advance for all patient appointments. This position will address the needs of the ACCESS cohort participants and at various events within Harris County.Duties and Responsibilities:

  • Responsible for providing ongoing case management services for participants enrolled in all ACCESS cohorts; works with participants across all cohorts to enhance stability and overall well-being by addressing specific needs that may have contributed to the identified vulnerability; assists with internal and external referrals for medical, substance use, mental health, parenting, employment, housing needs, and other needs related to social determinants of health.
  • Information sharing amongst safety net departments, community-based organizations, and any additional stakeholders; provide continuous feedback to the Care Coordination Team during case conference; maintains case records and documentation as appropriate across all cohorts; assists in the designing and implementation of key program components.
  • Maintains a calendar of outreach activities, including community events, workshops, appearances, assists with data collection and preparation of the community outreach and KPIs across all cohorts; prepares timely and accurate reports indicating progress toward program goals and outcomes.
  • Conduct site/home visits as needed to support all cohorts in completing initial assessments on new participants and provide follow-up to existing participants; ensure the safety and basic needs of the participant are met in an appropriate and caring manner; care coordination duties as well as conducting navigation and eligibility assistance for participants of various programs; performs outreach, enrollment activities.
  • Identifies participant compliance barriers across all cohorts, still ensuring quality of care coordination delivery; work in a team environment, fast passed environment, be flexible, be able to adapt to sudden changes, be trustworthy, be reliable, be empathetic.
  • Greets and guides patients through the ACCESS Harris registration process; assists with creation of singular eligibility document in alignment with ACCESS Harris Safety Net Collaborative.
  • Ensures the completion of participant paperwork and eligibility: accompany participants to medical, housing, and other needed appointments, and any other in-field support requested; communicate with members of the community, colleagues, and participants effectively and efficiently; collects consent forms/paperwork and maintains a list of all pre-registered participants for events.
  • Complete all pertinent program documents within designated timeframes; duties may include intake, enrollment, case management, monitoring, group facilitation, resource referrals, etc. effectively manage crisis situations through assessing, deciding, and acting in accordance with standards and laws in emergencies involving the health and safety of participants being compromised.
  • Responsible for establishing a strong working relationship with managers and all employees and attend regular internal and external agency meetings.
  • Other duties as assigned, including special tasks involved in responding to an emergency event.

Education:
  • Bachelor's degree from an accredited college or university majoring in Public Health, Public Relations, Social Work, or a related field.
Experience:
  • At least one (1) year of working in Case Management or Case Management experience.
  • At least one (1) year working with a variety of populations and servicing social needs.
Licensure/Certification:
  • Valid Driver’s License (TX upon hire)
Computer Proficiency:
  • Proficient in Microsoft Office Suite software, including 365 programs.
  • Must be comfortable learning new computer software and guiding others through its use.

  • May be required to travel which may include transporting clients, participating in community outreach programs, and attending meetings and training opportunities within Harris County.
Work Environment:
  • This role routinely uses standard office equipment such as computers, phones, iPad, photocopiers, filing cabinets, and fax machines.
  • Staff may make site visits using personal vehicle.
  • Staff in this position should possess a high level of tolerance for this emotionally demanding and stressful work while at the same time being supportive and understanding of the stresses being experienced by both participant and staff.
Physical Demands:
  • Frequently required to sit or stand, use manual dexterity, speak, listen, hear, and write.
  • May occasionally lift and move up to 25 pounds and occasionally lift to 50 pounds.
