Job Summary: The RN clinical manager shall perform necessary coordination of clinical and managerial activities as directed by the Director of Nursing to accomplish the objectives of the Clinical Management of the residents

Job Qualifications:

● Current MN License as an RN

● Experience working the long-term care setting favored.

● Preference given to experience in MDS/RAI or clinical reimbursement.

● Work effectively with minimal supervision, strong communication skills, and ability to lead a group of people in completing tasks.

● Knowledge of nursing assessments, resident care plans, and state and federal long-term care regulations.

● Ability to communicate well with residents and families explaining medical needs/changes related to resident’s care as well as advocate for the proper services to be provided.

● Meet the physical job demands of the position

Hours of Work: Hourly Position. Works weekends as necessary.

Duties:

● Accepts inquires and provides tours to potential residents and their families

● Maintains good relations with physicians, clinics, social worker, etc.

● Have current and updated brochures/pamphlets available to advertise and market the facility

● Provides effective communication with other providers (i.e. Medicare, Supplemental Insurance Carriers,

RAI Auditors, Physicians, Pharmacists)

● In charge of resident assessments

● Reports Vulnerable Adult issues to their director supervisor as necessary; initiates documentation of & investigation of suspected VA concerns.

● Visits with residents routinely regarding cares, activities, menus, plan of care etc.

● Responsible for admission and discharge assessments.

● Ensures any grievance is documented and reported to either the Social Service Director or Director of Nursing.

● Ascertain staffing adequate for the day

● Review 24-report and review documentation f/u

● Ascertain q 2h T&P done and heals floated for all residents who require T&P

● Rounds

o Make certain no chemicals are left unlocked or in resident’s rooms

o Check tube feedings & pumps

o Check O2 and BiPAP/CPAP/Pulse Oximeter

o Check foley for positioning/dignity and leg straps

o Monitor incontinence care

o Personal item labeling and storage

o Environmental cleanliness

o Infection control

o Assess grooming – nail care, hair care, oral care, bath schedule, appropriately dressed

o Ensure podiatry care provided as indicated

o Call lights answered timely

o Dignity and privacy maintained

o Restraints applied properly and released q 2hours

o Residents up timely and rooms made up by 10:00am

● Check labs – ordered, drawn, results received and MD notified, results on chart

● Ascertain CP’s are updated as indicated – as indicated by 24 hour report, I/A reports, new MD orders

● Review 24 hour report; ascertain that MD and RP notified as indicated, new orders transcribed properly, new orders implemented, nursing documentation complete and appropriate, care plan updated as indicated.

● Ensure that triple check policy is followed each month with the implementation of the monthly

POS/MARs/TARs (3 people looking at it)

● Check MARs and TARs daily for documentation

● Check ADL documentation daily

● Bath schedule adhered to

● Admission checklist completed on all new admissions – flu/pneumovax/tb testing completed as indicated and vaccination form in chart.

● Weekly skin assessments completed

● Establish schedule for monthly summaries and check compliance and adequacy of documentation; repeat for risk assessments

o Fall risks

o Skin Risk

o B&B

o Safe Smoking

o Wheelchair Assessment

o Hydration

● Monitor meals:

o Resident’s hands washed before each meal

o Assign coverage with NAs and LNs

o Residents receive correct meal and served sequentially

o Alternatives offered

o Adaptive devices correctly used

o Assistance provided as indicated

o Special dietary requirements followed: i.e. thickened liquids

o Meal consumption accurately documented

o Ascertain that in-room dining is properly monitored and meals delivered timely

● Make rounds daily to interview residents for concerns/issues and assess care delivery

● Immediately initiate investigation and f/u on all allegations of abuse/neglect

● Participate in all high-risk meetings and IDT Care Plan meetings

● Ascertain that narcotic counts are completed shift to shift

● Check Med rooms for fridge temps, E-kits, dated medications

● Ascertain that med carts and treatment carts are locked when staff not present

● Coverage for wings for NAR during breaks

● Check weight list daily for potential weight loss/gain.

● Make certain that no chemicals are left unlocked or in resident rooms. (Rounds)

● Ascertain that all incontinent residents have peri-wash and barrier cream in bedside stand

● Observe peri-care routinely

● Develops a good working relationship with the physicians, county social workers, discharge planners, vendors and other agencies

● Responsible for insuring D.O.N. is notified of changes in resident status, including falls, medication errors, and Vulnerable Adult issues.

● Demonstrates the ability to assess and evaluate resident needs and coordinate nursing care activities so

that these needs are met throughout the department.

● Supports or participates in the following programs: Resident Care Conferences, Admission/Discharge Planning, Quality Assurance, Utilization Committee, Fire and Safety, Infection Control, Employee Health Program, Medication Review, and will formally participate in the evaluation of each of the above listed programs as it relates to resident care and overall departmental activities.

● Completes other duties as designated

● Has a basic understanding of how the RAI/MDS process directly impacts the budget through the setting of rates and reimbursement.

● Communicates significant changes to D.O.N and Administrator (i.e. significant changes in case mix, case mix level, changes in facility rates annually)

● Participate in the control of cost-effective services that include proper use of equipment, appropriate use of supplies, and effective, efficient organization of resident care duties.

● Has a basic understanding of insurance benefits and reimbursement for services provided.

● Responsible for the building; cleanliness, repairs, replacement of fixtures and equipment; submit requests to Maintenance Department in a timely fashion to request any repair or maintenances.

● Aware of disaster plans for the facility and orientates all staffs who are left in charge of building to these plans and procedures.

● Responsible that storage areas are kept neat and organized to promote a safe environment.

● Monitors that employees are completing proper documentation as necessary; if not reviews with employees and attempts to remedy any issues & if ongoing discusses with Director of Nursing.

● Monitors day-to-day performance of staff and responds appropriately to both positive and negative outcomes

● Acts as a professional role model to other staff for carrying out procedures, documentation, leadership, and problem-solving skills.

● Assists other staff (licensed staff and CNA staff) to be redirected in correct procedures as becomes aware of deficient areas, or directs concerns to DON.

● Wears Name Tag at all times

● Reports to work on time each day; both staff and direct supervisor are aware of schedule in advance.

● Responsible for development and implementation of an effective, thorough orientation program for new employees. Provide proper training to others on documentation and when appropriate clinical assessments.

● Seeks opportunities to maintain education, knowledge and skills; state and federal regulations, best clinical practices, and geriatric nursing practices

Job Types: Full-time, Permanent

Pay: $34.00 - $42.00 per hour
