Overview
We are looking for a Social Worker MSW to join our team. This position reports to the Administrator or Executive Director and is responsible for evaluating and developing a plan of care personalized to fit the patient’s emotional and social needs. The Social Worker MSW provides direction and supervision of the Social Worker BSW as required and when involved in patient’s plan of care.
- Assesses the patient’s social and emotional state as it relates to his or her illness or injury, needs for care and his or her response to such treatment and adjustments to care
- Provides any appropriate action to obtain available community resources to assist in resolving issues that may be impeding the patient’s recovery
- Instructs/counsels patients and families in treating and coping with social and emotional response connected with illnesses. Provides ongoing assessment of patient and family needs and responses to teaching
About You
- Master’s degree in social work from a CSWE School
- Social Worker licensure in the state of practice
- Minimum of 2-3 years’ experience as a Social Worker in a clinical care setting, home health and/or hospice preferred.
