DAY-TO-DAY

This full-time social work position in human services works during the day.

As a Mental Health Case Manager with our nonprofit, you serve as part of our Case Management Support Program (CMSP) to help individuals that have substance abuse disorders, are in transitional housing, have a severe mental illness, or have other complicated behavioral health issues. You provide counsel to clients on the treatment and rehabilitation process as well as advise them of their rights and responsibilities. As needed, you help clients coordinate a manageable treatment and work schedule and accompany them to appointments. You also help clients collect documentation for social security and housing applications as well as provide advocacy services throughout the application process.

Additionally, you provide community-based services in a variety of settings in a flexible manner, which may include clients' homes, treatment facilities, transitional housing sites, or shelters. Some of the services you provide involve helping clients understand and explore their needs, strengths, interests, aptitudes, experiences, capabilities, career goals, and family situations. You continuously update and maintain your client's comprehensive service plans and address any additional support services that are needed such as housing, legal services, medical, domestic violence, educational, or employment services.

Based on substance abuse assessments, you provide referrals to treatment or harm reduction services. You maintain ongoing contact either face-to-face or by phone with clients to promote and encourage their compliance with our programs. Using your keen eye for detail and psychology expertise, you identify early warning signs of crisis and conduct timely relapse intervention as needed. You provide outreach to clients struggling with program participation and address any underlying issues preventing their success. REQUIREMENTS FOR A MENTAL HEALTH CASE MANAGER

  • Bachelor's degree in social work or a related field such as psychology, vocational rehabilitation, or occupational rehabilitation from an accredited institution
  • Credentialed Alcoholism and Substance Abuse Counselor (CASAC)
  • 2 years of case management experience OR related training or experience
