Evangelical Childrens Home is Hiring a Transitional Living Specialist Near St. Louis, MO

Essential Duties and Responsibilities Include the following. Other duties may be assigned.

Client Health:

  1. Ensure that the health needs of all clients are met.
  2. Schedule appropriate health related appointments for clients
  3. Facilitate transportation to appointments as
  4. Managesclientadherencetomedical
  5. Provides support navigating the healthcare system.
  6. Teaches and builds life skills through meeting healthcare needs.
  7. Provides regular observation of client health needs and follows up in
  8. Assists with implementation of standard as well as specialty appointments and required follow-up.
  9. Teaches access to healthcare at appropriate levels.
  10. Creates a living healthcare file.

Demonstrationof Active Participation in Treatment Team:

  1. Maintain close communication with youth workers.
  2. Review documentation from service plan
  3. Support clients through shared problem solving and clear decision
  4. Communicate effectively with co-workers and
  5. Working effectively and professionally while under pressure
  6. Provide treatment team with health updates on program
  7. Maintains communication with program client's families as needed regardingclient

Medical Files:

  1. Conducts reviews of client medical files including client's medical history
  2. Maintains files including all relevant health information and documentation of
  3. Files paperwork on a weekly

Documentation:

  1. Perform and maintain update health assessments on program
  2. Document case notes of all medical appointments, and indirect/direct contact with clients or medical Provide necessary documentation by:

Completing progress notes in FamCare after providing life skills instructions with residents.

  1. Maintaining daily log of events and transports.

Community:

  1. Serves as a representative of the program in the community and interacts with various medical professionals/
  2. Arranges for community support services for program clients that are in
  3. Makes official contact with medical health professionals and others associated with program
  4. Utilizes community resources to meet the needs of program
  5. Works with outside agencies to ensure quality of care for youth and their
  6. Provides referral assistance to program clients regardingtheir health

Client Support:

  1. Supports clients in all areas including health and mental health
  2. Work with volunteers and outside agencies that provide support to
  3. Supports and monitors clients' progress in all aspects including health, education, employment,

Group Home (Secondary Role):

Plan and execute Life Skills lessons:

  1. Executing hands-on individual and group learning opportunities and activities developed with supervisor (cooking, cleaning, hygiene, fitness, time and money management, safety, etc.)
  2. Assisting in employment processes (finding, applying, interviewing, maintaining and appropriately leaving a job) in coordination with Life Skills Department
  3. Provide overall supervision and accountability of youth by:
  4. Provides supervision and ensuring security of youth
  5. Accounts for whereabouts of all youth
  6. Provide staff coverage at the home when needed to support the needs of the youth
  1. Provide on-going assistance to youth by:
  2. Ensuring all basic needs are being met (food, medical needs, safety, cleanliness, etc.)
  3. Providing transportation to youth consistent with service plan
  4. Assisting and instructing residents in utilizing community resources such as the bus system, local libraries, etc.
  5. Developing positive, professional relationships and boundaries with youth
  6. Providing additional assistance and supervision as directed by the service plan or by your supervisor
  7. Providing crisis intervention as necessary to include calling police, ambulance, or fire department when needed.

Administrative/Professional:

  1. Performs administrative responsibility while carrying out the duties of the on-call
  2. Aidintheeventofacrisisor
  3. Maintains minimum agency standards for continuing education and training
