Easterseals Blake Foundation is Hiring a Family Support Coordinator (Tucson) Near Tucson, AZ

Description

Family Support Coordination aims to support families with children birth to age 5 with more complex needs by assisting them in understanding, navigating to and accessing services that promote family well- being. Family Support Coordination is an intensive, short term support which builds upon family strengths and needs to develop goals that promote family self-reliance.

Essential Duties and Responsibilities:

  • Recruit new families to enroll in Family Support Coordination services (e.g. share information with families in the community, distribute materials to child care providers, libraries, apartment communities and pediatric offices).
  • Accept referrals, contact family within two (2) business days to complete the Protective Factors Concrete Supports Pre-Survey; then use the results to determine if Family Support Coordination is a good fit based on the family’s needs.
  • Schedule an initial home visit within five (5) business days, at a time convenient to families, to explain the program components and enroll families into the program.
  • Develop a written service plan inclusive of individualized goals with outlined steps and timeframes towards successful completion, and that are based on the information provided by the family.
  • Refer and connect families to critical and timely services that meet their needs.
  • Obtain consent from families when sharing any information on behalf of the family with other providers.
  • Provide families with information and support to help connect them with needed services (e.g. support them in making an initial telephone call to the service, explain the enrollment process, visit the agency with the family, etc.)
  • Network with other providers in the Pima South region to promote streamlined connections to services for families, including Family Resource Centers.
  • Work closely with Family Resource Centers to provide family activities when needed.
  • Assess family strengths and resources, including informal support systems.
  • Complete all program documentation requirements and maintain confidential, individual family files.
  • Maintain an effective and professional working relationship with families and referral sources.
  • Work with families to determine when transition from services is appropriate based on the service plan goals, and provide information and resources to promote stability.
  • At program exit, deliver the Protective Factors Concrete Support Post Survey to families, as well as provide families with a copy of the service plan and any information that they may need to help them address potential needs as they arise.
  • Ensure fidelity to First Things First Standards of Practice.
  • Function as an enthusiastic, competent, contributing member of EBF staff.
  • Use Reflective Supervision effectively, accept feedback and share experiences working with families.
  • Attend and participate in all required staff meetings.
  • Participate in FTF monitoring activities and contribute to the development and implementation of continuous quality improvement plans.
  • Comply with State, Federal and Other Relevant Regulations/Requirements.
  • Report suspected neglect/abuse to the Department of Child Safety and supervisor, complete required documentation.
  • Comply with privacy laws and requirements under HIPAA regulations
  • Follow all safety procedures required by EBF.

Requirements

Education:

  • Bachelor’s or Master’s degree in social work, early childhood development, early childhood education, family studies, or a related field.
  • Must be bilingual in Spanish.

Experience:

  • 1 year of experience in providing services for high risk/high needs populations.
  • Knowledgeable about resources available to families in the community and how to connect them to supports and services to address needs.

Regulatory:

  • Clean criminal background check and the ability to obtain and maintain an AZ DPS Level 1 fingerprint clearance card.
  • Must have a valid, unrestricted Arizona Driver’s License, good driving record, and be at
  • least 21 years of age as required by corporate fleet policy.
  • Reliable Vehicle
