East Valley Family Services is Hiring a Family Support Specialist (Case Manager) Near Las Vegas, NV

JOB DESCRIPTION

The Family Support Specialists are to maintain our mission by offering clients the resources they require to become and remain self-sufficient, healthy and socially responsible. Completes full family assessments to determine the services needed.

DUTIES AND RESPONSIBILITIES:
Program:

  • Completes a comprehensive family assessment on each client
  • Establishes a case plan with families, as deemed appropriate
  • Informs client of proper documentation needed for each application, helps collects documents and makes appropriate copies to distribute as needed
  • Assists clients with application completion and logs time and date of submission in files
  • Offers clients referrals to other agency programs, as appropriate
  • Encourage clients to partake of EVFS educational programs
  • Conducts weekly follow-ups with clients inquiring about and documenting their progress
  • Advocates with agencies when client approval appears to be a problem
  • Keeps supervisor informed, weekly, concerning families with extensive issues and needs
  • Maintains client information in files and in the HMIS database system within 48 hours of seeing clients
  • Assist with the bagging of TEFAP food for client distribution

General:

  • Maintains good working relationships with all community partners that are established with East Valley Family Services’ referral processes
  • Attend meetings and trainings, as required
  • Follows established guidelines and reports any incidents of abuse/neglect of a child to Supervisor and to Child Protective Services intake departments
  • Ability to be a team player
  • Is pro-active in the marketing and public relations aspect of East Valley Family Services
  • Observes all protocols and maintains confidentiality
  • Participates in events held by, and for the benefit of the agency
  • Conducts outreach events weekly and/or monthly. Additionally, on some evenings and weekends
  • Other duties as assigned

QUALIFICATIONS: High School Diploma or GED or high school level writing and reading skills. Work experience or other related education in this field as well as bilingual ability is a plus. Six months experience working with at-risk families Valid driver’s license Ability to clear a background check. Demonstrates competency with basic computer skills and programs. The ability to work in a team environment The ability to communicate effectively Completion of mandatory reporting training and code of ethics training

Job Type: Full-time

Pay: From $33,000.00 per year
