Lifespan Home Health is seeking a “Case Manager” to help support our growing Austin, TX office.
We are an established and stable home care agency that provides in-home, non-medical care to the elderly and children and adults with disabilities so they can remain safe and comfortable in their home, with their family, and in the community.

The Case Manager manages provider services programs (i.e. STAR PLUS, Primary Home Care, Family Care, STAR Kids) and provides DIRECT personal care as needed to patients and families in accordance with Agency policies and scope of practice.
