DESTINATION HEALTHCARE SERVICES

JOB DESCRIPTION

TITLE: Program Manager

EXPERIENCE: Three years of experience working as a team member with people with disabilities. Knowledge of state of Tennessee DIDD rules and regulations; 2 years of supervisory experience required.

  • Must be 18 years of age or older
  • As applicable, must possess a valid driver's license, good driving record and liability car insurance as defined by Destination Healthcare Services
  • Must satisfactorily complete computerized state required training, first aid/CPR and Medication
  • Administration Training Certifications as required by regulations.
  • Must be able to pass a background check as defined by regulations and policies. Ability to do daily standing, bending, lifting, walking.
  • Staff who have direct contact with or direct responsibility for people using services must be able to effectively read, write, and communicate verbally in English and read and understand instructions, perform record-keeping duties and write reports.

EDUCATION: Bachelor’s degree in human services or social science from a regionally accredited institution in human services or related discipline

DEPARTMENT: Program Services

REPORTS TO: Executive Director

SUPERVISES OTHERS: Residential Service Managers

RESPONSIBILITIES: Abides by and demonstrates the company’s Mission, Vision, and Values through both behavior and job. o Interacts frequently and positively with people supported by the organization. o Monitors/implements/supervises delivery of services and supports o Train staff on ISPS of people supported by the organization o Foster open and supportive communication with people supported, DSP staff families, conservators and guardians. Assure that all medical, medication and health issues are addressed o Follows agency’s procedures to promote optimal health for people supported o Monitors to ensure that all sites provide services in accordance with DIDD regulations and the agency’s policies and procedures o Implement and follow-up on all support services recommendations o The program coordinator also serves as the agency’s Complaint Resolution and

Compliance Officer, ensuring the complaints from people supported, families, staff, and other stakeholders are addressed, tracked and completed in a satisfactory manner.

SUPERVISION/TRAINING

  • Monitor staff performance management, including evaluations and developmental plans, disciplinary actions, etc., for all RSMs.
  • Supervise staff training and development through teaching, acting as a role model and guiding staff on how to appropriately work with and interact with consumers, other staff, families and guardians.
  • Present and implement all agency policies and procedures.
  • Participate in the interviewing and hiring of new staff.
  • Inform supervisor as needed about pertinent issues.
  • Be available for on call schedules. Carry the on-call phone on a rotational schedule; respond while on-call to program needs including covering open shifts.

MEETINGS/FUNCTIONS

  • Attend/chair as needed manager, supervisory and staff meetings; ensure dissemination of all information to staff and others.
  • Coordinate/attend as needed all consumer related meetings (IHP, IDT, case manager, circle, house, family, etc.); ensure all necessary follow-up is completed.
  • Participate in special projects and committees.

REPORTS/PAPERWORK

  • Ensure completion/submission of all required daily, bi-weekly, monthly, quarterly and annual reports as scheduled.
  • Routinely review all required reports, logs, mail, requests, etc. as needed, maintaining an organized filing system.

FINANCE

  • Monitor all aspects of program financial procedures including monitoring of utility use in accordance with agency and Division of Intellectual and Developmental

Disabilities' policies.

  • Monitor all aspects of consumer financial procedures in accordance with agency and
  • Division of Intellectual and Developmental Disabilities policies. Assure the proper and prudent expenditure of all funds.
  • OSHA standards.
  • The employee agrees to cooperate with Agency and DIDD Staff during any inspection or investigation.
  • Perform additional duties as required by the nature of the position or as requested by supervisor.

JUDGEMENT/INITIATIVE: Must be capable of functioning independently with minimum supervision. Must be able to initiate innovative programs and use good professional judgment in ongoing decision-making.

COMMUNICATION SKILLS: Excellent oral and written skills (in English) required for ongoing communication with consumers, agency staff and community agencies.

WORKING CONDITIONS/HAZARDS: The behavior of consumers may change dramatically without obvious cause, which may create, at times, hazardous situations.

Pay: $40,003.57 - $50,606.45 per year
