As a Medical Social Worker in this position, you will be responsible for implementing the patient's plan of care, assessing and providing patient reports and assisting family members with patient self-care, and coordinating home health services.
We are currently seeking a self-motivated, dynamic, and caring individual to join us as a Medical Social Worker in Tucson, AZ.

What we are looking for:
- Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work.
- Master’s Degree
- A minimum of one (1) full year of supervised post-certification experience in a healthcare setting.
- Home health experience preferred.
