Arizona is Hiring a Social Worker, Hemophilia Team Near Tucson, AZ

  • Work cooperatively as part of a multidisciplinary Hemophilia team to provide direct comprehensive behavioral health services to patients and families diagnosed with congenital bleeding and clotting disorders, this includes working extensively with medical faculty.
  • Conduct patient need assessments prior to clinic visit so that information can be conveyed to clinical team.
  • Conduct interviews with patients and families during annual clinic visits to evaluate psychosocial and cultural needs.
  • Prepare psychosocial assessments that are used in establishing management goals and chart them in electronic medical record and Cerner software.
  • Maintain appropriate documentation in an electronic record system consistent with ethical guidelines, state statutes, and internal policies.
  • Provide consultative and referral services to patients and families for outside community resources and entities for assistance (financial, vocational, schools, transportation, housing, insurance, and medication).
  • Work with families in securing resources for assistance with finances, living, transportation, and medication assistance.
  • Advocate for patients within the medical and community setting.
  • Interact with and maintain membership of various local and national community support groups that educate and provide assistance to persons/families with congenital bleeding disorders.
  • Provide short and long term therapy/counseling in conjunction with the behavioral health counselor based on informed and relevant practices to reinforce patient's strengths, improve wellbeing, and assist them with managing the stress of living with chronic illness.
  • Respond to crisis situations and provide additional assessments as needed.
  • May organize group counseling and/or programs or retreats that assist patients with psychosocial needs.
  • Collaborate with the HTC patient financial counselor to ensure that patients are receiving accurate information about health insurance options and are educated about choices for post-hospital care (rehabilitation/skilled nursing facilities).
  • Assist with the organization and attend hemophilia camp each year.
  • Annually visits each school that has a patient in attendance to provide education and support services to educators regarding the psychosocial needs of students living with a lifetime chronic disease.

Knowledge, Skills, and Abilities

  • Ability to work and coordinate with other team members while operating independently.
  • Possess excellent written and verbal communication skills.
  • Skill in Office 365.
  • Knowledge and understanding of basic social work principles.
  • Knowledge and ability to work in medical setting.
  • Knowledge of educational institutions and schools as it pertains to 504 plan or IEP for younger patients.
