Denton County MHMR Center is Hiring a TCOOMMI COC Worker Near Denton, Tx

Under direction of the Director of TCOOMMI Services and within federal, state and local guidelines provides moderately complex case management work. Work involves developing and maintaining long-term contact with clients, client families, and service providers for medical, social, educational, and related service needs. May train others.

Provide Continuity of Care services to individuals with mental illness and criminal justice issues. Responsibilities include planning and linking needs to community resources and monitoring the delivery of services to ensure identified needs are addressed. Works under moderate supervision, with limited latitude for the use of initiative and independent judgement.

Education, Training and Experience (including licensure and certification)

Bachelor's degree in a human services field is required. Must qualify as a QMHP (Qualified Mental Health Professional), which includes a minimum of 31 college hours in human services courses. Minor in Criminal Justice or double major with a human services degree and Criminal Justice preferred. Six months experience in criminal justice field required, one year experience preferred.

REGISTRATION, CERTIFICATION, LICENSURE OR OTHER QUALIFICATIONS:

  • All job specific training must be completed within 30 days of hire.
  • Must maintain a valid Texas driver's license, auto liability insurance and a driving record acceptable to the Centers insurance requirements.

Knowledge, Skills and Abilities

  • Knowledge of community resources; of case management principles, objectives, standards, and methods; and of program policies and procedures.
  • Ability to assess clients' needs, to coordinate clients' services, to communicate effectively, and to train others.
  • Understanding and practice of Trauma Informed Care principles.

Essential Duties include the following. Other duties may be assigned.

  1. Organizes and tracks TCOOMMI clients services including maintaining spreadsheets; maintains TCOOMMI Web App; obtains and submits required paperwork for financial/billing purposes, including monthly client caseloads to billing, clinic, and consumer benefits; ensures clients attend all appointments to comply with COC and TCOOMMI requirements; meets with ICM clients at Center, jail, parole office or other community setting to provide training/psychosocial rehab services when available.
  1. Administers TCOOMMI assessment to all TCOOMMI clients in time frame required by TCOOMMI contract; sets up services for clients transferring from TCOOMMI services in another county or providing referral for all TCOOMMI county to county transfers from Denton County.
  1. Completes all TCOOMMI, Probation, Parole reports in timely fashion; updates all paperwork, meets with parole officer on monthly basis to discuss client's treatment progress and provide monthly report to both Center staff to complete comment sections and to Parole Officers for their records; requests psychological evaluation be faxed to Parole Officers for mental health caseload requirements on monthly basis.
  1. Locates community resources for clients to assist with individual needs and completes follow up with client to determine success or lack of success with accessing resources.
  1. Interviews clients, reviews records, and acts as member of the Interdisciplinary Team to evaluate effective client treatment; obtains medical paperwork (EMR) needed to ensure COC for clients transitioning from jail/prison; formulates programs for treatment that consider client needs as viewed by client, family members and service providers, prepares client for transition into other MHMR services or community at end of 90 day TCOOMMI services; works as liaison between client and clinic staff and advocates for client as needed, both in Center services and with other community agencies.
  1. Monitors client to evaluate success of therapy and adapts treatment as needed 100% of the time; monitoring is conducted in the consumer's environment, which may include jail, home, work, etc.; determines or refers consumers for financial assistance and eligibility.
  1. Meets with clients face to face monthly to determine needs and assist with accessing resources to meet those needs; completes all closeouts and discharges in a timely manner.
  1. Provides verbal and physical PMAB intervention correctly as necessary; provides CPR/First Aid as needed and appropriate until medical professionals arrive or as directed by a licensed medical professional; complies with staff development and is responsible for scheduling and completing DCMHMR required training; attends all meetings, functions, trainings, etc. for TCOOMMI via interoffice trainings and modules or via TCOOMMI/TCA meetings (as funds are available).
  1. Complies with all aspects of MOU mentioned above including scheduling all TCOOMMI appointments, maintaining communication with Parole officers and reporting compliance of all clients; receives all probation and parole referrals and schedules all appointments for intake and any required follow up appointments, informing parole/probation officer, client, and intake staff of appointments; ensures all paperwork is submitted for billing purposes; completes all recommendations for services for parole/probation clients and submits in timely manner to referring officer and monitors placement of clients in services to ensure accountability of client needs
  1. Visits the Denton County Jail at least one (1) time per month to provide continuity services and to advocate for TCOOMMI clients currently incarcerated; visits jail to complete 46B follow up visits and determine client needs during incarceration and after release and fills out all required CARE paperwork.
  1. Establishes and maintains relationships with other community agencies to build a referral network; participates in team building activities by attending scheduled team meetings, utilizing clinical supervision as needed and maintaining positive communication with colleagues and supervisor.
