Decision Point LLC, a therapeutic residential and outpatient care program designed to treat adults with chemical dependency, co-occurring disorders, and trauma, is seeking a Behavioral Health Case Manager. Experience in the treatment industry preferred. Must possess good written and verbal communication skills, effective scheduling, and coordination skills. Must work well in a crisis setting and be willing to learn new crisis intervention techniques.
Requirements:
Work Shift: Tuesday -Saturday
Bachelors Degree
1 year of working in a behavioral health setting. Case manager experience in a treatment setting is preferred.
Must maintain the highest level of confidentiality
Effective scheduling and coordination skills
Must be open-minded, flexible, and willing to take direction
Must possess good verbal and written communication skills
Must work well in a crisis setting and be willing to learn new crisis intervention techniques
Qualifications for position:
Maintain client caseload
Perform client care management tasks necessary to meeting with clients on a regular basis to ensure the highest level of customer service
Assist clients with the achievement of treatment goals
Maintain accurate and complete records of correspondence, and services rendered to client
Assist client with exit strategy plans and aftercare
Communication and coordination of client care
Liaisons and communicates to the family members, marketers, and referent sources as needed
Run peer support classes with clients on caseload
Employee is preferred to have experience with recovery and be familiar with community fellowships such as AA, NA, SMART Recovery, Celebrate Recovery, etc.
Attends clients staffing meetings weekly
Other duties as assigned

The work environment may include exposure to outdoor weather conditions
It May involve exposure to and handling of potentially bio-hazardous materials
Employees with potential access to protected health information must comply with all procedures and guidelines governed by HIPAA.
PHYSICAL DEMANDS:
Must have reasonable ability to stand, walk, sit, use hands to finger, handle, feel, reach, climb or balance, stoop, kneel, crouch or crawl, talk, hear, taste, smell, lift 50 lbs.
