Job Title: Patient Care Coordinator

We are seeking a highly organized and detail-oriented Patient Care Coordinator to join our team. The ideal candidate will be responsible for scheduling and confirming all new patient PT and medical visits, confirming all new patients completed intake paperwork prior to the initial visit, and confirming all pain procedure patients. The Patient Care Coordinator will also schedule and confirm all TBI patients, confirm all patients have scheduled follow-up visits, and confirm and enter all new patient information and correspondence in the company CRM.

Responsibilities:

• Schedule and confirm all new patient PT and medical visits
• Confirm all new patients completed intake paperwork prior to initial visit
• Confirm all pain procedure patients
• Schedule and confirm all TBI patients
• Confirm all patients have scheduled follow-up visits
• Confirm and enter in all new patient information and correspondence in company CRM
• Attempt to reschedule all no-show and cancelled patient visits that do not have rescheduled visits
• Communicate with the front desk staff to ensure all patients have follow-up visits
• Enforce scheduling protocol and rescheduling protocol with the front desk staff
• Delegate and manage Spanish-speaking patients to Spanish-speaking staff for scheduling
• Participate in weekly company metrics meetings for procedure scheduling and new patient stats

Requirements:

• High school diploma or equivalent
• 2 years of experience in a medical office setting
• Excellent communication and organizational skills
• Proficient in Microsoft Office and CRM software
• Ability to work independently and as part of a team
• Fluent in English and Spanish (preferred)
