Community Care Center is proud to offer a fun, exciting program including outings, high-tech gaming, and various events and classes to cater to our residents’ psychosocial needs.
Position Summary:
The primary purpose of this MDS nurse position is to perform assessments of new patients, develop treatment plans for residents that comply with MDS guidelines. Treatment plans must be submitted accurately, timely, and be compliant with current regulations. This opportunity is for a 100 bed skilled nursing facility with a Subacute Unit.
- Must possess a Nursing degree from an accredited nursing program (required).
- Must be a Licensed Vocational Nurse (LVN) or Registered Nurse (RN) in the State of California (required).
- Must possess MDS Certification from an accredited program (highly preferred).
- MDS experience in a Long-Term care setting (highly preferred)
- Must be certified in CPR.
- IV certification preferred
- Must maintain current licensure in good standing during employment with this facility in accordance with the State Board
