Community Passageways is Hiring a Case Manager (Recovery Navigation) Near Seattle, WA

The Recovery Navigator Program (RNP) provides community-based outreach, intake, assessment, and connection to services and, as appropriate, long-term intensive case management and recovery coaching services, to youth and adults with substance use disorder, including for persons with co-occurring substance use disorders and mental health conditions, who are referred to the program from diverse sources and shall facilitate and coordinate connections to a broad range of community resources for youth and adults with substance use disorder, including treatment and recovery support services.
The case management position is critical to the CP-Recovery Navigation Program. The case manager will provide direct services to a caseload of approximately 25 individuals. Case managers provide outreach, field-based intensive case management, long-term management with supportive services, individualized intervention plans, and referrals for participants in collaboration with LEAD partners, service providers, housing providers, and other community-based organizations.

Essential responsibilities

The employee must be able to perform the essential functions of this position satisfactorily and, if requested, reasonable accommodation will be made to enable employees with disabilities to perform the essential functions of their job, absent undue hardship.   • Engage participants on the street and at social service provider facilities to establish a trusting relationship and offer services. 
  • Assess participants for severity of chemical dependency, housing status, and mental health needs. Determine needs for services (e.g., medical, mental health) depending on participant’s treatment readiness. 
  • Assist participants in gaining access to a variety of programs (e.g., healthcare, SSI, VA, community referrals, mental health/CD assessments)
  • Assist participants with gaining and maintaining housing. 
  • Develop and implement alongside participants an Individualized Service Plan which addresses the needs, goals, and strengths of the participant.
  • Develop and maintain a working relationship with LEAD partners/community based organizations including: Sobering Center, DHSH workers, chemical dependency treatment providers, mental health providers, shelter providers, landlords, detox, assessment centers, protective/representative payees and other community programs that support participants. 
  • Develop and maintain collaborative relationships with Seattle Police Department, King County Sheriff, and City of Seattle Prosecutor’s office. 
  • Provide advocacy and support for participants within the criminal legal system, including court appearances and written communication. 
  • Attend regularly scheduled operational work group meetings and clinical consults.
  • Identify gaps and barriers in community resources and advocate for systemic changes.
  • Develop and maintain participant files for assigned caseload according to program, contract, and state requirements.
  • Additional duties as assigned.

  • Understanding of harm reduction along with a demonstrated passion for serving individuals experiencing homelessness and co-occurring disorders is required.

  • Be proficient with mainstream office software such as G Suite, email and calendar scheduling tools

Requirements

Preferred Skills/Experience 

  • 1 year of Case management experience or in a similar position 
  • Connection and understanding of community resources and systems
  • Lived experience with the criminal legal system - policing, jail, state supervision. We believe in a peer-led model and value the lived experience of those most impacted by the system. 
  • 1 year of providing advocacy and support for participants within the criminal legal system, including court appearances and written communication.

Requirements 

  • Driver’s license, personal vehicle, and car insurance. Travel throughout the community and visiting participants in person are required for this position.
