Position Summary: The Community Health Worker Works as integral member of the Population Health Care Management team while managing the day-to-day responsibilities of the community health worker service delivery model, which includes the evaluation of program outcomes and individual staff productivity and performance. Additionally, the Community Health Worker directly delivers or effectively connects patients to appropriate health and wellness resources, public benefit programs, and other needed social services/community resources in a timely and culturally appropriate manner, with the goal of addressing social determinants that negatively affect self-management of chronic disease. This is a full-time position. 

Salary - This position starts at $52k and up based on experience. Indeed provides their own estimated salary calculator and is not affiliated with COH's range.

Highlighted Duties and Responsibilities:

  • Works with patients to develop an individualized care plan that includes short and long-term goal setting, with a particular emphasis on social needs, barriers to care, behavioral health support, medication management, and chronic disease self-management
  • Provides education to patients, families, and caregivers regarding health conditions, medication management, healthy lifestyle and chronic disease self-management
  • Provides and tracks referrals to clinical and community resources, including housing and job training programs, support groups, legal services, childcare, etc..
  • Reduces gaps and delays in care by ensuring patients complete appointments. Proactively works to resolve patient-identified barriers to care, makes reminder calls, schedules appointments, arranges transportation, contacts patients following missed appointments, etc.
  • Contributes to programmatic support activities, including reporting, quality improvement initiatives, outreach to eligible patients, and creation of programmatic materials
  • Facilitates patient empowerment and quality of life by promoting educated, independent patient choice on all aspects of care. Helps patients develop competency in understanding their personal medical information and health conditions
  • Ensures that patients receive timely and culturally sensitive support services and interventions, and that care is coordinated with patients and across the multidisciplinary team
  • Counsels patients on the appropriate utilization of health services in order to avoid unnecessary utilization of emergency services
  • Secures patient information and maintains patient confidence by completing and safeguarding medical records and keeping patient information confidential

Requirements

Minimum Qualifications:

  • High School Diploma/GED required; Associate Degree or higher preferred.
  • Two years work experience; prior experience in public health or human services and/or in community health worker or similar role preferred.
  • Excellent interpersonal skills, ability to show empathy, and ability to interact professionally and gain trust with culturally diverse individuals.
  • Bilingual in Spanish or Amharic speaking strongly preferred.
  • Prior experience working with community resources and social services in DC.
  • Ability to understand and apply clinical, social, and environmental aspects of chronic disease, treatment, and self-management.
  • Ability to handle confidential information with discretion and professionalism.
  • Computer skills and proficiency in the use of handheld mobile device technologies.
  • Ability to travel between COH sites or relocate to a different COH site on a full or part -time basis required

For over 40 years, we have provided healthcare, housing, and supportive services for under-resourced, underserved and people experiencing homelessness in Washington, DC. As a Federally Qualified Health Center, we provide medical, dental, emotional wellness, and care coordination services for the whole family at three locations in DC. Community of Hope also strongly emphasizes maternal and child health, with midwifery practice and the only free-standing birth center in DC. In 2020, Community of Hope provided about 28,400 medical visits, 7,000 dental visits, and 10,000 behavioral health visits for about 11,000 patients. Community of Hope provides community walk-in COVID testing and COVID vaccines. Community of Hope is also one of the largest providers in DC of housing and support services for families and individuals experiencing homelessness.
