The Family Nurse Practitioner reports to the Director of Family Medicine regarding administrative, provider relations and other day to day guidance. Assigned to supervising physician under the Texas Medical Board for medical consults and abides by appropriate clinical protocols. Facilitates clinical issues between clinicians and provides guidance, both written and verbal between centers and Call center to ensure patient needs are met. Provides patient care via Telemedicine, phone consults and addresses medication refill requests as needed.
DUTIES AND RESPONSIBILITIES:
1. Ensures call center staff operates regarding published clinical guidelines. Ensures adherence to existing Policies and Procedures to facilitate seamless patient care. Serves as liaison to physicians and other departments on issues such as referrals and walk-ins. A key role of this clinician is to triage, and address patients concerns that avoid unnecessary Emergency Room visits and to facilitate seamless patient resolution for valid patient concerns.
2. Triages, diagnose, treats and/or refers patients assigned on an out-patient or telemedicine basis. Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions consulting with supervising physician as needed.
3. Maintains a problem-oriented health record for each patient seen in accordance with the Center’s established Health Informatic Management policies and procedures to include providing appropriate and legible documentation. Completes thorough documentation of each patient’s visit in the electronic health record and ensures encounters are closed within 24 hours in accordance with the Center’s established Health Information Management policies and procedures.
4. Assures continuity of care by providing medical services in a timely basis as per Clinic schedule within the Center’s scope of practice and the Nurse Practitioner’s training.
5. As needed will Review and report lab results, address medication refill requests from patient and pharmacies, additionally release and review imaging reports for patients that maybe under their or other clinicians care in a timely manner.
6. Provides excellent internal/external customer service via telephone consult, Telemedicine, face-to-face contact to assist patients with their health care needs.
7. Will provide clinical oversight over clinical support staff engaged in activities and procedures, as appropriate.
8. Follows established departmental and clinic policies, procedures, and objectives; continuous quality improvements objectives and safety, environmental, and/or infection control standards.
9. Participates in specified health promotion, education and/or prevention programs as needed i.e. diabetes collaborative/health fairs etc.; attends and participates in clinic meetings / departmental meetings and other clinic functions as required by the Center.
10. Performs other related duties as assigned.
JOB QUALIFICATIONS:
Master’s Degree from an accredited university in a Nurse Practitioner Program
Previous experience as Family Nurse Practitioner highly preferred
Must have current unrestricted license to practice as a Nurse Practitioner from the Texas Board of Nursing
Annual continuing medical education as required by board specialty
Must be registered and have current DEA
Experience with Telemedicine highly preferred
Bilingual in English and Spanish highly preferred
Certified in Basic Life Support (BLS), American Heart Association

REQUIRED PHYSICAL AND MENTAL ACTIVITIES AND REQUIREMENTS:
Finger Dexterity: Using fingers to make small movements such as typing or picking up small objects. Normal fine and gross motor control of fingers and hands.
Talking: Frequently conveying detailed or important instructions or ideas accurately, clearly, or quickly.
Hearing: Able to hear average or normal conversations and receive ordinary information; hear whispered voice at five (5) feet.
Repetitive Motions: Frequently and regularly using the wrists, hands, and fingers.
Visual: Visual acuity necessary for the proper evaluation of a patient and to prepare or inspect documents or other materials. Ability to accurately view computer monitors
Physical: Sitting 20-30% of time and standing, walking 70-80% of time. Lift up to 25 lbs; push and pull up to 45 lbs. Able to bend at the waist, twist and turn along axial plane.
