Job Description: *
Position Summary *(see program descriptions below):

  • Under the direction of the Program Manager of CUL CMHA Community Connections, this position is responsible for coordination of all program activities; program service delivery; ensuring all services meet contract, funding, and accreditation requirements, and are consistent with the mission, vision, and values of the Columbus Urban League. This position is responsible for delivering client-centric services and meeting performance requirements that are consistent with the mission of the Columbus Urban League.

Service Coordination - Columbus Metropolitan Housing Authority
Service Coordination at Sawyer/Trevitt will support residents in targeted areas of their lives, conducting comprehensive needs assessments, creating individualized service plans based on resident’s needs, connecting residents to services to meet their specific needs, and connecting regularly with residents until they successfully complete their identified goals.

Duties & Responsibilities:

  • Under direct supervision of the Program Manager, plans and ensures effective program activities and services; provides prevention programming to selected and indicated populations, monitors activities for efficacy; ensures client services, documentation and data collection have integrity, are accurate and are compliant with funding, contract and/or accreditation requirements; proposes changes to immediate supervisor, as necessary.
  • Ensures program is meeting stated outcomes and/or goals by preparing monthly and/or quarterly reports and statistical analysis of stated outcomes as required by the program manager. Provides recommendations for continuous improvement.
  • Works within the CULs integrated model of service delivery, with other CUL departments to ensure all customers can access services relative to their needs.
  • At minimum, maintains a full program calendar as required by funding guidelines and agency standards, guide customers through program services.
  • Provides supervision and guidance to Community Health Worker(s) assigned to the site-based program.
  • Reviews and signs off on case notes created by Community Health Workers in the appropriate database systems on a weekly basis to ensure service quality and performance measures are being met according to grant requirements and for invoicing and billing.
  • Manages relevant database systems as required by the CUL and funders.
  • Participates in regularly scheduled operations meetings as required; prepared, able to contribute as necessary.
  • Serves as a reference resource for CUL sponsored services, and coordinates resources with other CUL programs.
  • Participates in committee meetings, staff meetings, staff trainings, and other activities contributing to the effective, efficient operation of the agency as needed.
  • Supports the program manager to ensure continuity in services and adheres to program expectations and the CUL mission.

Other Duties as Assigned
SALARY RANGE: $55,000 - $60,000

Required Experience: *
Job Requirements Education and Experience: *

  • Bachelor’s degree in human services, or related field required or minimum three (3) years’ experience.
  • Must meet Ohio Department Mental Health & Addiction Services requirements to implement prevention services (RA, LSW, OCPSA, etc.).
  • If not already credentialed, must become a Prevention Professional Registered Applicant with the Ohio Chemical Dependency Professionals Board within one (1) month of hire, and OCPSA within six (6) months of hire.
  • Three (3) to five (5) years of post-secondary experience in counseling, case management or human services work will be considered.
  • Excellent case management skills and ability to work with adolescents on an individual and small group basis.
  • Excellent customer services skills with professional demeanor and networking skills.
  • Excellent written and oral presentation skills.
  • Ability to manage time effectively and to work independently.
  • Ability to meet aggressive goals for department programs.
  • Professional business attire and appearance required.
