About the Job
We are looking to hire an ICM Case Manager for our Intensive Case Management (ICM) program in Jackson County, Oregon. The ICM Case Manager provides outpatient recovery support services to clients with severe and persistent mental illness. The ICM Case Manager provides community-based case management and skills training services to clients living with serious mental illnesses in multiple settings including hospitals, residential treatment facilities, in the individual’s home, or other community settings. Services may continue on a short or long-term basis with the purpose of maintaining the client safely in the community, without hospitalization.
Members of the ICM team work closely with hospitals and other community partners, individuals, outpatient service providers, and Jackson Care Connect to identify and implement alternatives to psychiatric hospitalization, which can include a variety of service plans designed to help stabilize clients who are experiencing severe psychiatric symptoms.
Work Schedule:Monday through Friday, 8:00am – 5:00pm (Full Time, Day). Case managers are required to work on a weekly on-call rotation.
What You’ll Make
$27.20 - $30.00 per hour DOE/Credentials.
Additional 5% Language Differential offered for Bilingual or Multilingual candidates (Spanish/English desired).
About the Program
ColumbiaCare provides Intensive Case Management (ICM) Services, which are high-quality, evidence-based services that are flexible: a community-based package of care that includes assessment, strengths-based treatment planning, and recovery-oriented case management that helps identify the individual’s service needs and establishes and maintains community support systems and service coordination. This position would be based out of Central Clinic, in Medford, Oregon.
ColumbiaCare Services is a qualifying employer for the Public Service Loan Forgiveness (PSLF) Program. QMHA employees arealso eligible to apply for the Oregon Behavioral Health Loan Repayment Program.
What You’ll Need
Must be eligible for Qualified Mental Health Associate (QMHA) registration. Eligibility requires 1 out of 3 Criteria below:
If not already registered as a QMHA through the Mental Health & Addiction Certification Board of Oregon (MHACBO), we will assist you in registering within 30 days of hire.
Driving is an essential function of this position. Successful applicants MUST also have a valid Oregon driver’s license, maintain a safe driving record, and have the ability to pass a DHS criminal background check and a pre-employment drug screen.
Physical Requirements: This position requires the ability to frequently talk, listen, use hands and fingers, sit, stand, walk, reach; as well as lift, carry, push, and pull up to 40 pounds. It may require the ability to occasionally climb stairs and ladders, bend, stoop, squat/kneel, and perform other physical tasks as applicable. In addition, you must successfully complete crisis de-escalation and CPR trainings, which require performing various physical, hands-on maneuvers and techniques. Reasonable accommodations can be made.
