About the Job
We have an opening for a Veterans GPD Case Manager in Southern Oregon. Grant and Per Diem (GPD) Case Management works directly with Veterans by assisting them transition into qualified housing from GPD and HCHV programs, retain their housing once placed, and further develop the skills needed for successful independent living in the community. Other responsibilities include assisting Veterans with obtaining and maximizing benefits, helping Veterans interface with landlords and access public transit and services, connecting Veterans to services that help them maintain sobriety and improve their mental health, negotiating conflicts, and resolving problems. The GPD Case Manager's work occurs at the Veterans Resource Center, other ColumbiaCare locations, and out in the community, including client homes. Frequent travel is required, and safe driving is an essential part of this position.
Work Schedule: Monday through Friday, 8:00am – 5:00pm (Full Time, Day)
What You’ll Make
$6,000 - $7,250 per month DOE/Credentials.
Additional 5% Language Differential offered for Bilingual or Multilingual candidates (Spanish/English desired).
About the Program
The Veterans Resource Center connects Veterans with the services they need. The VRC is focused on the overall wellness and reintegration of Military Veterans of all eras. With a primary focus on housing assistance, case management, and outreach, we take a holistic approach to addressing the individual needs of each Veteran. We invite all community partners to utilize the VRC as we combine efforts and collaborate to help those we serve. The VRC serves as a single-entry point to provide the help Veterans need by forming a coordinated network of community partners.
ColumbiaCare Services is a qualifying employer for the Public Service Loan Forgiveness (PSLF) Program. QMHP employees are also eligible to apply for the Oregon Behavioral Health Loan Repayment Program.
What You’ll Need
Driving is an essential function of this position. Successful applicants MUST also have a valid Oregon driver’s license, maintain a safe driving record, and have the ability to pass a DHS criminal background check.
Physical Requirements: This position requires the ability to frequently talk, listen, use hands and fingers, sit, stand, walk, reach; as well as lift, carry, push, and pull up to 40 pounds. It may require the ability to occasionally climb stairs and ladders, bend, stoop, squat/kneel, and perform other physical tasks as applicable. In addition, you must successfully complete crisis de-escalation and CPR trainings, which require performing various physical, hands-on maneuvers and techniques. Reasonable accommodations can be made.
