JOB SUMMARY
Under the general supervision of the Associate Director of Clinical Services, the Care Coordinator will be responsible for providing a safe and nurturing environment for children and youth between the ages of 6-18. The Care Coordinator will oversee the intake process for all agency programing, including T-Suites, by reviewing referrals and other intake information, conducting interviews, and welcoming clients to the unit. The Care Coordinator serves as a liaison for clients in accessing and coordinating support services.
MINIMUM QUALIFICATIONS:1. Education, Training, and/or Experience:- A Bachelor’s degree in Social Work or related field; and
- A minimum of two (2) years' experience in counseling, childcare, social work, and/or in providing therapeutic support services to children and families; and/or
- An equivalent combination of education, training, and experience.
2. Certifications, Licenses, and Registrations:- Successful completion of all HR recruitment prerequisites including proof of current TB test upon hire
- A valid Driver’s License, appropriate driving record with appropriate insurance coverage (as required by agency policy and/or insurance carriers)
PREFERRED QUALIFICATIONS:- A Master’s degree in Social Work or related field
- Licensed by the State of Ohio Counselor and Social Work Board as a Licensed Social Worker (LSW), or a Professional Counselor (PC)
ESSENTIAL FUNCTIONS, DUTIES AND RESPONSIBILITIES

1. Welcome children entering the facility upon entry and admission
2. Coordinates services with other agency programs and staff
3. Collects reviews and evaluates referral material and client information
4. Interviews clients and prepares intake documents, as necessary
5. Interviews placement agency workers
a. Interview other personnel associated with the placement process
b. Requests additional information from referring agencies, as necessary
6. Coordinates all youth placements with program staff.
7. Maintains intake documents and logs of all referrals
8. Maintain case records and documentation in compliance with regulations and standards
a. Completes necessary paperwork
9. Provides care, support, attention, and a positive, therapeutic atmosphere to all children in the department to which assigned
a. Maintain consistent, firm, and fair relationships with all children
b. Serves as an appropriate, positive role-model
10. Attends and participates in all meetings, staffing’s, SARS and court hearings related to assigned clients
a. Works cooperatively with staff as a team member in assessing client’s adjustments and total program effectiveness
11. Provides physical and emotional security for the children through intervention, correction, and re-direction of negative behavior when:
a. Behavior is damaging to self;
b. Behavior is damaging to others
c. Behavior may result in damage to property
12. Assists with managing the physical needs of the children including health, hygiene, and physical comfort (food/clothing/shelter)
13. Educating clients and families about community resources to help address any social determinant of health needs.
14. Provides formal and informal reports on behavior, attitude, and general disposition of assigned children, including Incident Reports, restraint remediation sheets, time-out reports; Department logs; Behavior Rating Scales; Primary sheets
15. Attends conferences and meetings
16. Participates in the planning, supervision, and provision of therapeutic and recreational activities
17. Transports children as necessary including, but not limited to medical appointments, activities, school
18. Assist with serving as a community resource for local agencies and others interested in the program.
ADDITIONAL DUTIES & RESPONSIBILITIES
1. Maintains and/or increases knowledge, skills, and abilities
2. Attends conferences, meetings, and/or committees including supervisory sessions, general staff meetings, training, department, and treatment plan meetings, evaluations, and triage/psychiatric /medical treatment conferences
3. Performs other duties as assigned and/or needed.
4. Assists with providing transportation of clients, as needed
Knowledge, Skills, Abilities & Personal Characteristics:- Knowledge and support of agency mission, goals, policies, and procedures
- Basic knowledge of mental health administration including applicable department rules, regulations, functions, procedures, and policies
- Knowledge of therapeutic milieu philosophy, techniques, and quality standards
- Knowledge of methods/techniques for intervention, correction, and re-direction of negative behavior
- Basic knowledge of the physical and emotional needs of children including health, hygiene, consistency, supervision, and recreation
- Knowledge of applicable forms/documentation
- Organizing, coordinating, and prioritizing techniques and methods
- Ability to provide a positive, therapeutic and safe environment for children
- Ability to perform physical restraints safely and correctly
- Ability to establish a rapport with clients
- Ability to maintain appropriate and required confidentiality ensuring the integrity of sensitive information (written and/or verbal)
- Ability to carry out written and oral instructions
- Ability to work independently
PHYSICAL DEMANDS described here are representative of those that must be routinely met by an employee to successfully perform the essential duties of this position.
- Ability to frequently work with and around household cleaning agents and other chemicals.
- Physical capacity for frequent bending, stooping, and lifting.
- Ability to lift to 25 pounds and move heavy objects.
- Work involves some exposure to physical risks such as physical restraint, unpleasant odors, and/or loud noises
- Ability to perform job responsibilities in a facility or residence with multiple levels and without elevators or mechanical transportation.
