As a Care Coordinator, you will provide care for high-risk patients in our department. This role requires someone who can multitask in a fast-paced environment. A Care Coordinator will be responsible for encouraging wellness andreducing emergency department visits and readmissions. Our department is eager to teach and welcome new people to the team. Your primary duties will include but are not limited to:
• Performing phone triage and appointment follow-ups.
• Reviewing treatment plans with various caregivers.
• Scheduling labs.
• Working closely with schedulers.
