As a Care Coordinator, you will provide care to patients in our department. A Care Coordinator is responsible for encouraging wellness, reducing preventable emergency department visits and readmissions by educating patients. This department is eager to teach and welcome new people to the team.

Work Experience:
Three to five years of nursing experience required.

Follows Standard Precautions using personal protective equipment as required for procedures.
