A $3,000 hiring bonus is being offered for this position. Hiring bonuses shall only be for new hires of original employment or reemployment as defined in Human Resources Policy No. 5.0. 

• $750 will be awarded in the first paycheck.
• $1,000 will be awarded upon completion of the six (6) month interim performance evaluation with an overall rating of “Meets Expectations”.
• $1,250 will be awarded upon completion of the employee’s one (1) year probationary period and a performance evaluation with an overall rating of “Meets Expectations”.

This position is also eligible to receive a $6,000 annual stipend to be paid in installments over the 26 pay periods.

The City of Virginia Beach Department of Human Services is actively seeking to hire a dedicated candidate with experience providing medical/clinical services to adult consumers with serious mental illness and co-occurring disorders for the LPN - Medication Room Nurse position with the Program of Assertive Community Treatment (ACT). 

Duties:

• Designated as the ACT medication room nurse, responsible for following the guidelines necessary to maintain the Virginia State board of pharmacy controlled substance registration and ensuring compliance with medication room policies.
• Provide medical/clinical services to adult consumers with serious mental illness as well as co-occurring consumers.
• Perform nursing duties to include, but not limited to administration of oral and IM medications.
• Package and prepare medications for delivery and administration.
• Assist in the provision of medical services, such as delivering and monitoring medications, scheduling and accompanying consumers to medical appointments.
• Transcribe medication orders to the MAR and provide oversight of MAR completion to ensure accuracy and avoid errors in the program.
• Provide guidance to teammates as appropriate to correct MARs.
• Link with external stakeholders to include pharmacies and medical providers.
• Collaborate with medical providers and requesting medical records.
• Provide education/counseling regarding medications and relevant nursing problems.
• Provide supportive counseling in 1:1 to assist consumers with the development of improved health/wellness, coping, problem-solving and communication skills related to treatment goals.
• Provide training and support with activities of daily living, performs crisis intervention, and facilitates prescreening/hospitalization when necessary.
• Assist the ACT nurse manager and other team members. 
• Maintain electronic health and clinical records in accordance with regulatory standards and ensures protection of consumer rights and confidentiality.
