Aging and Disability Services division (ADS) manages the continuum of care that supports older individuals and those living with a disability, and their families. The mission of Aging and Disability Services is to develop a community that promotes quality of life, independence and choice for older people and adults with disabilities in King County. Our goal is to promote healthy aging, ensuring that our communities are great places for people to grow up and grow old.

Position Description:
The City of Seattle Human Services Department is seeking Registered Nurse Consultant to join the Aging & Disability Services Division. The successful candidate will join a team of enthusiastic, energetic, and dedicated colleagues who provide home-based case management services to vulnerable adults throughout King County. The RN Consultant helps support individuals with complex medical conditions and will collaborate with medical providers and other key partners to maximize health care resources and positive health outcomes for clients.

The successful candidate will love working with clients may include those who have difficulty sustaining basic living needs due to their physical, emotional, behavioral or cognitive status, and physical or emotional neglect or abuse. We're looking for a highly professional RN Consultant who is also comfortable working with ethnically diverse adults who are eligible for Medicaid in home personal care.
The RN Consultant will be a key member of the client’s healthcare team, providing consultation to Case Managers in the Aging and Disability Services Case Management Program. RN Consultants with ADS do not provide hands-on clinical care to clients, but do make home visits to observe/assess skin breakdown, verify treatment plans and make appropriate referrals.
Client population includes older individuals and adults with disabilities, living at home, who have unstable health conditions and risk factors including multiple physical and/or mental health problems, long-term care needs, and/or drug/alcohol issues. Clients may include those who have difficulty sustaining basic living needs due to their physical, emotional, behavioral or cognitive status, and physical or emotional neglect or abuse. The client population for the program are ethnically diverse adults who are eligible for Medicaid in home personal care. These positions report to a Case Management Program Supervisor.
Registered Nurse Consultants provide medical expertise, input and consultation to a team of social work case managers whose clients are at high risk of residential placement or hospitalization due to unstable health conditions or caregiver/provider situations. Client population consists of frail, elderly, disabled adults receiving Medicaid funded services with multiple risk factors including physical and/or mental health problems, long-term care needs, drug/alcohol issues, behavioral or cognitive status, and physical or emotional neglect or abuse.
