Aging and Disability Services division (ADS) manages the continuum of care that supports older individuals and those living with a disability, and their families. The mission of Aging and Disability Services is to develop a community that promotes quality of life, independence and choice for older people and adults with disabilities in King County. Our goal is to promote healthy aging, ensuring that our communities are great places for people to grow up and grow old.

As the state-designated local Area Agency on Aging (AAA) for Seattle and King County, ADS work in partnership with King County to: 

  • Improve the health and quality of life for older adults and adults with disabilities
  • Connect seniors and adults with disabilities with helpful resources.
  • Provide help and support for caregivers.

Position Description:

The City of Seattle Human Services Department is seeking Registered Nurse Consultant to join the Aging & Disability Services Division. The successful candidate will join a team of enthusiastic, energetic, and dedicated colleagues who provide home-based case management services to vulnerable adults throughout King County. The RN Consultant helps support individuals with complex medical conditions and will collaborate with medical providers and other key partners to maximize health care resources and positive health outcomes for clients.

Do you love providing compassionate care to older individuals and adults with disabilities, living at home, who have unstable health conditions and risk factors including multiple physical and/or mental health problems, long-term care needs, and/or drug/alcohol issues? The successful candidate will love working with clients may include those who have difficulty sustaining basic living needs due to their physical, emotional, behavioral or cognitive status, and physical or emotional neglect or abuse. We're looking for a highly professional RN Consultant who is also comfortable working with ethnically diverse adults who are eligible for Medicaid in home personal care.

The RN Consultant will be a key member of the client’s healthcare team, providing consultation to Case Managers in the Aging and Disability Services Case Management Program. RN Consultants with ADS do not provide hands-on clinical care to clients, but do make home visits to observe/assess skin breakdown, verify treatment plans and make appropriate referrals.

Client population includes older individuals and adults with disabilities, living at home, who have unstable health conditions and risk factors including multiple physical and/or mental health problems, long-term care needs, and/or drug/alcohol issues. Clients may include those who have difficulty sustaining basic living needs due to their physical, emotional, behavioral or cognitive status, and physical or emotional neglect or abuse. The client population for the program are ethnically diverse adults who are eligible for Medicaid in home personal care. These positions report to a Case Management Program Supervisor.

Registered Nurse Consultants provide medical expertise, input and consultation to a team of social work case managers whose clients are at high risk of residential placement or hospitalization due to unstable health conditions or caregiver/provider situations. Client population consists of frail, elderly, disabled adults receiving Medicaid funded services with multiple risk factors including physical and/or mental health problems, long-term care needs, drug/alcohol issues, behavioral or cognitive status, and physical or emotional neglect or abuse.

  • Work closely with case managers to develop a plan of care.
  • Coordinate with other providers such as primary care, specialty, mental health and home health.
  • Meet with clients in community or their homes to assess their needs.
  • Conduct skin observation for pressure injuries and other wounds and make recommendations for care.
  • Provide education and training to caregivers, clients and staff.
  • Make referrals to other services such as Adult Day Health and medication management.
  • Use Care Transitions strategies to help clients avoid unnecessary re-hospitalization.
  • Use a laptop to enter your notes and assessment.
  • Use your keen nursing eye to identify gaps in care plans and help case managers to fill those gaps.
  • You may be asked to carry a small caseload of clinically complex clients.
  • Find opportunities to advocate for clients and dismantle barriers. 

Education: 

  • A Bachelor of Science in Nursing

AND

Experience: 

  • Two years of clinical nursing experience with elderly and/or disabled adults OR
  • Three years of clinical nursing experience with any age group

Or a combination of education, training and/or work experience which provides the ability to perform job responsibilities

AND

Certification/License:

  • Washington State Registered Nurse license.
  • Current State of Washington driver's license or evidence of equivalent mobility.

  • You are experienced using electronic medical records to record client information and can learn new systems.
  • You have a second language fluency in Spanish, Russian/Ukraine, Somali, Punjabi and/or Bosnian with ability to effectively communicate orally and in writing in English and second language;
  • You have an understanding of Medicaid systems.
  • You can utilize standard office software such as Outlook, Word, SharePoint and Excel.
  • Bonus points if you have skill and experience with computerized CARE assessment program.

Your clinical skills:

  • You have extensive experience with chronic illnesses in a variety of settings, including residential and home health care as well as family practices and clinics.
  • You are experienced using Motivational Interviewing to assist clients with behavior change.
  • You have experience with Care Transitions strategies.
  • You are able to coordinate care plans with social services, physicians, home health agencies, hospitals, and health industry providers;
  • You are experienced working with elderly or disabled adults who have unstable health conditions and risk factors including multiple physical and/or mental health problems, long-term care needs, and/or drug/alcohol problems.
  • You have strong clinical assessment and planning skills.
  • You can stage a wound and make appropriate treatment recommendations.
  • You have the ability to manage a small caseload of clinically complex clients.
