We are seeking an individual with a Hospice Heart. A well-independent, organized, service-centered, caring, empathetic, compassionate person who can educate, manage symptoms, and support patients, families, and loved ones.
We serve individuals and their families during the advanced and final stages of a terminal illness. Our loving and compassionate team comes alongside patients, families, and other loved ones to facilitate comfort and dignity at the end of life.

General Responsibilities:
Under the direction of the Director of Patient Care and our Medical Director, this position will provide nursing care and hospice case management to hospice patients and families.
Implements nursing interventions and coordinates the patient and loved ones' interdisciplinary Plan of Care.
Provide excellent clinical care and support to relieve patients' physical pain and help patients and loved ones find peace at the end of life.
Provide care and support to both patients and their caregivers with regularly scheduled visits throughout the week.
Provide continuous education, training, and medication management for symptom management and pain relief to patients and families.
Requirements for this position are:
RN Nursing License in the State of Nevada
Valid Driver's License with an acceptable driving record
At least one year of nursing experience, preferably in oncology, med surg, hospice, or home care.
At least 3 years of computer experience.
Must be able to navigate a computer, electronic medical records system, email and other digital tools and systems
Involves daily travel to patients' homes and facilities.
This position will have a zoned service location, most likely the Reno/Sparks area, but must be willing to travel to other areas if required or needed.
The RN Case Manager adheres to the regulatory process required by the State Board of Nursing and Federal Agencies as required.
