Clinical Evaluator (RN or LPN), Per Diem

Center for Living & Working, Inc., (CLW), is a 501(c)3 non-profit agency founded in 1975 by a core group of young adults with disabilities, and located in Worcester, Massachusetts. CLW is a consumer directed, multi-programmed, multifaceted agency, dedicated to the empowerment of all persons with disabilities to live as independently as possible.

CLW is looking for a Clinical (RN or LPN) Evaluator to join our Personal Care Management (PCM) Department. The Clinical Evaluator is responsible for:

• Evaluating the physical and cognitive functional status of consumers in their homes,
• Making recommendations to maximize their safety and independence for the Personal Care Attendant (PCA) and/or Massachusetts Rehabilitation Commission (MRC) Home Care Assistance Programs,
• Obtaining, completing and submitting all necessary documentation as required by CLW to support the PCA Program so successful applications can be made on a timely basis. (i.e., evaluations, billing, invoice, etc.)

Requirements:

• 1-3 years of experience as a RN or LPN in either a hospital or community nursing environment.
• Must be a licensed Registered Nurse or licensed Practical Nurse (Massachusetts)

• Own transportation to and from Consumer meetings.
• Bi-lingual Preferred

Compensation: Pay is per evaluation. Home visits, charting of evaluations and associated tasks can take between 1.5 to 4 hours to complete. Mileage reimbursement is paid separately. Skilled, organized and efficient Clinical Evaluators can earn an average of $30 to $50 per hour. Additional incentives and bonuses are periodically offered but are not guaranteed.
