Center Health Care Services is Hiring a COMMUNITY ENROLLMENT LIAISON Near San Antonio, TX

GENERAL SUMMARY
The Community Enrollment Liaison (CEL) typically works within a multidisciplinary team to provide an array of direct (face-to-face), indirect, and community setting clinical services to adults in a person-centered trauma informed and individualized focus approach. Individuals served may have multi-system involvement such as Criminal Justice, Child and Adult Welfare, Veterans, and Homelessness. This position serves the community by providing CHCS enrollment services in various settings:
Hospitals
Services are delivered in hospital settings with patients in the emergency room and inpatient psychiatric units. Community Enrollment Liaisons in this setting conduct any CHCS intake function to include MCOT, CMOT, Adult Behavioral Health and Child Behavioral Health, as well as expedited screenings for CHCS private psychiatric bed and state hospital bed access.
Detention and Community Based
Services delivered by the Community Enrollment Liaisons in this setting include conducting CHCS enrollment functions, assisting with New Generation Medication funding eligibility for inmates while incarcerated, and deliver services in the community setting for inmates that were recently released from jail to include psychosocial rehabilitative services and case management.
ESSENTIAL DUTIES & RESPONSIBILITIES
Essential functions are the basic job duties that an employee must be able to perform, with or without reasonable accommodation.   • Assists hospital staff with discharge coordination activities such as making referrals to appropriate facilities (Josephine Recovery Center, San Antonio State Hospital, Crosspoint, etc.)
  • Attends daily hospital staffing meetings to obtain updates on the patients’ clinical treatment, discharge status, referral determinations. Shares treatment updates and/or collateral to the patient’s clinical treatment team at CHCS.
  • Collaborates with a field based Care Manager (Transitional Liaison) to engage patients who have been non-compliant.
  • Completes Crisis follow up within 24 hours.
  • Completes end of day reports to team regarding respective facility.
  • Completes monthly reports for CHCS and Southwest Texas Regional Advisory Council (STRAC).
  • Conducts informational group sessions to educate patients regarding CHCS services.
  • Conducts Mobile Crisis Outreach Team (MCOT) Hospital Assessment for expedited screenings into a CHCS state-funded private psychiatric bed or state hospital bed.
  • Connects established CHCS consumers’ Care Manager to the patient’s clinical treatment team, schedule a hospital discharge appointment with the patient’s prescriber, and update clinical assessments as needed.
  • Coordinates coverage for planned absences in advance with supervisor.
  • Engages patients into outpatient services upon discharge from hospital. The Transitional Liaison can connect with the patient until the first outpatient appointment. Identifies the appropriate level of care, and facilitates referrals to specialized programs at CHCS. This will typically take place within 72 hours of discharge.
  • Ensure clinical documentation is submitted in accordance with CHCS standards, and contract specific requirements.
  • Examines patient’s Social Determinants to Health (SDoH) by completing Signify (electronic health platform) authorizations and making appropriate community referrals and linkage to identified needs.
  • For eligible patients, complete enrollment into CHCS outpatient mental health services and prior to discharge from the hospital. Completes registration, financial assessment, consent forms, mental health clinical assessments, recovery plan, and provides a hospital discharge appointment with a CHCS prescriber.
  • Monitors and tracks patient compliance with follow-up appointments, and to conduct engagement efforts to identify barriers to outpatient services. The Transitional Liaison can perform engagement activities for those patients who do not show to their appointments.
  • Participates in Centralized scheduling process.
  • Participates in supervision and development opportunities including individual supervision, group supervision, and meetings/trainings as assigned.
  • Provides direct clinical services and assessments. Develops treatment plan for consumers.
  • Performs other related duties as required.
Additional Liaison Duties within Detention and Community Based Locations
  • Collaborates with the Transitional Liaison to engage patients who have been non-compliant; i.e. miss their post release enrollment appointment with jail liaison.
  • Communicates daily with DHCS staff to identify inmates/patients who will be releasing from jail or have returned from the state hospital and assist in identifying appropriate referrals, and provide treatment updates and/or collateral to the patient’s clinical treatment team at CHCS.
  • Completes enrollment into CHCS outpatient mental health services for eligible patients, prior to release from jail and post release when needed in the community. Completes registration, financial assessment, consent forms, mental health clinical assessments, recovery plan, and provides initial and follow up appointments as needed.
  • Connects established CHCS consumers to Care Manager and prescriber post release for follow up/continuity of care.
  • Cooperates with University Health in complying with all standards, rules regulations and policies of The National Commission of Correctional Health Care, The Texas Department of Juvenile Justice, and applicable policies of University Health and requirements of the Joint Commission (JC).
  • Enrolls inmates/patients into outpatient services prior to or upon release from jail. Identifies the appropriate level of care, and facilitates referrals to specialized programs at CHCS.
  • Helps coordinate transportation for inmates being released with CHCS staff/drivers for transportation to clinics or home/community at time of release.
  • Monitors and tracks enrollments completed, return to jail arrests within 90 days of release for those enrolled in CHCS services, and inmates that received jail liaison services in the community within 30 days of release.
  • The liaisons would serve as the primary role to provide referral, linkage, and transition to the next level of care in partnership with providers and community partners. Interim case management would be provided post discharge until first follow-up appointment with prescriber and or case management. The Transitional Liaison would also identify appropriate patients for TRR/Specialty Services, CenterCare, or other community partners.
  • Provides community based services to fill gaps for those enrolled into or are established with CHCS outpatient services while waiting for initial visit with assigned care manager or prescriber. The Detention Liaison would also be responsible for enrollments with patients who are coming from The San Antonio State Hospital (SASH) being released to jail and then to the community. Enrollment would preferably occur before release from jail or in the community.
  • Performs other related duties as required.
MINIMUM ENTRANCE QUALIFICATIONS
Education and Experience
  • Bachelor’s degree from an accredited university or college in a Behavioral Health field such as Social Work, Psychology, Counseling or a related field; or Bachelor’s degree with at least 27 hours of Behavioral Health course work
Licenses or Certifications
  • Complete the Qualified Mental Health Professional Community Services (QMHP-CS) within six (6) weeks of hire or transfer Licenses or Certifications
OTHER JOB REQUIREMENTS
  • Must maintain a valid driver’s license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive your vehicle during company business
  • Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
  • Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
COVID-19 Mandate Requirements
Unvaccinated Staff options:
a. Get Vaccinated per the CMS Mandate.
b. Have a medical/religious exemption. Unvaccinated staff will need to test once a week at the employees own time/expense and must wear enhanced PPE to include an N95 face mask and a face shield.
c. Consumer facing staff DO NOT have the option to be unvaccinated without an exemption.
d. If the position is not consumer facing, staff members may continue to be unvaccinated, without an exemption and test once a week at the employee’s own time/expense and must wear enhanced PPE to include an N95 face mask and a face shield.
Vaccinated Staff options:
a. For employees who are vaccinated and consumer facing, we are continuing to require wearing an N95 mask.
b. For all other vaccinated employees, we are requiring a cloth, surgical or N95 mask be worn in all shared spaces.
c. Vaccinated employees may only go unmasked if in a single occupancy office.
PREFERRED QUALIFICATIONS
  • Bilingual (English/Spanish)
  • Master’s degree from an accredited university or college in a Behavioral Health field such as Social Work, Psychology, Counseling or a related field.
  • One (1) year experience providing similar services
  • Prior experience with or knowledge of trauma-informed care
