Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

We currently have an opening for a Navigator II for our Phoenix Outreach program (Transit Team).

Outreach Navigator/Engagement Specialists are expected to use the best possible practices for outreach, engagement and enrollment into services. The position will be working with a variety of people and groups to locate and engage community members who need behavioral health services, including, providing assistance with completing the applications necessary to receive benefits and treatment.”

Skills and Requirements

• Minimum of a HS Diploma or equivalent is required
• Minimum of 2 years of mixed behavioral and physical health work experience and/or 2 years of substance abuse/mental health recovery oriented service
• Minimum of 6 months experience as a Navigator or Peer Support Specialist
• The Navigator is an individual who is in recovery from substance use and/or General Mental Health for an appropriate period of time (greater than or equal to one year).
• Must have a valid AZ Driver's license with 39 month clear MVR
