Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.
We currently seek a Navigator II for our Supportive Services for Veterans and Families (SSVF) program at our Phoenix, Central City location.
The SSVF program provides rapid rehousing and homelessness prevention for veterans through a rapid rehousing intervention in an effort to quickly help the veteran access permanent housing following the SSVF Core Concepts designed to work toward ending veteran homelessness.
Navigators identify, engage, and establish relationships with Veterans while they are living on the streets and continue to provide support after they transition into housing. The Navigators work with Veterans to link them to whatever is needed to end their homelessness and sustain their housing stability.
The Navigator will also map out the most appropriate next steps for the individual who may need assistance in accessing various services after or before the individual has been housed. The Navigator works in conjunction as part of a multi-disciplinary team made up of other Community Agencies and stakeholders, and CBI Peer Support Services & Outreach Management Team. The Navigator will document and track required patient interactions according to federal, state, local and CBI policies.
