We currently have an opening for a Navigator II position at our SOR program, which provides services to people with Serious Mental Illness, including those with co-occurring substance use disorders, who are experiencing homelessness or at imminent risk of becoming homeless. This is done through assertive engagement and targeted outreach services.
The Navigator II plays a vital role on this multidisciplinary team striving to improve the outcomes for individuals suffering from homelessness, primarily caused by an undiagnosed and/or untreated mental health concern. Navigator II is responsible for the outreach and engagement of individuals suffering from homeless and identifying those struggling with an undiagnosed mental illness. The Navigator II is responsible for for the coordination of care activities and ensuring connection to ongoing services.

Skills and Requirements

• This position requires a BHT Certification, valid driver’s license with a clean MVR for 39 months. Has a verified general equivalency diploma (GED) or high school diploma with a minimum of 2 years of mixed behavioral and physical health work experience and/or 2 years of substance abuse/mental health recovery oriented service .

Navigator II’s are individuals in recovery from homelessness, mental health or substance abuse.
