Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness

We currently seek a Navigator II for our Permanent Supportive Housing program at our Phoenix, Central City location!

The primary role of the Navigator is working with individuals who are homeless and in of housing and support services. The Navigator serves as the bridge for the patient, in coordinating and monitoring RHBA and Non-RBHA community based services and facilitates delivery of these services by supporting the individual. The Navigator will work with individuals who have been determined as the most vulnerable homeless according to the coordinated using assessment tools and will be responsible for conducting outreach, engagement, case management, and ongoing wrap around supportive services. The Navigator will also map out the most appropriate next steps for the individual who may need assistance in accessing various services after or before the individual has been housed. The Navigator works in conjunction as part of a multi-disciplinary team made up of other Community Agencies and stakeholders, and CBI Peer Services & Outreach Management Team. The Navigator will document and track required patient interactions according to federal, state, local and CBI policies.

Minimum Requirements:

  • Master's degree or Bachelor's degree in a field related to behavioral health is preferred.
  • Is a Registered Nurse, or a Physician Assistant who is not working as a medical practitioner, and has a bachelor's degree and at least one year of full-time behavioral health work experience; or has an associate's degree and at least two years of full-time behavioral health work experience,

Or

  • Has a verified general equivalency diploma (GED) or high school diplomawith a minimumof 2 yearsof mixed behavioral and physical health work experience and/or 2 years of substance abuse/mental health recovery oriented service.
  • Recovery oriented service experience will be verified whenever possible however respect must be paid to confidential aspects of different community resources that are based in anonymity.
  • Must also have completed160 hoursof behavioral health, substance abuse or physical health training post high school education or via continuing education units through on-line learning management systems, college courses or conferences and seminars.
  • Requires a valid Arizona Driver’s License and 39 month clear MVR
