Catawba Valley Healthcare seeks a full time qualified professional/case managers with at least two year’s experience to work as part of a ACT team based out of Hickory NC. This position will provide case management services to the individuals who meet criteria and choose to participate in the Assertive Community Treatment team. This position will complete initial assessments with individuals and develop a treatment plan based off of needs identified in the assessment. The person hired will work as part of a multi-disciplinary team and will offer case management services including assistance in accessing resources, medication management, transportation. This individual will carry out rehabilitation and support functions that assist in treatment, substance abuse services, education, support and consultation to families, and crisis intervention under the clinical supervision of psychiatrist and team leader.

Must meet Qualified Professional criteria to include bachelor degree in human services and 4 years experience with SPMI population.

Certification in SA as CCS, CCAS, or CDAC required - licensure preferred.

Job Type: Full-time

Pay: $45,000.00 per year

• 8 hour shift
