Catalytic Health Partners is Hiring a Front Office Coordinator Near Phoenix, AZ

Entry Level Position

At Catalytic Health Partners (CHP) our Front Office Medical Assistants are dedicated to completing the backend work necessary to ensure our members are receiving great care. Your role will have you working with our providers in the field and with our members to make sure their needs are met. CHP works to exceed quality metrics set by AHCCCS and ensure our members receive great care around their physical health, behavioral health and social needs. The Front Office Medical Assistant works in our CHP office, providing great customer service to our members and providers while completing their daily tasks to support the initiatives of CHP. The primary responsibilities of this role include setting up transportation and making sure our members arrive safely to their appointments, communicating hospital admissions to the field providers, and assist the other clinical support staff in the office.

Work Schedule:

  • Monday through Friday for the following shift: 6:00am to 3:00pm with a 1-hour lunch

Description

  • At Catalytic Health Partners (CHP) our Front Office Medical Assistant are dedicated to supporting our members to meet our field team in their home focusing on their social, behavioral, and physical needs. Medical Office Specialists will also be working via phone, text messages, and email with members to learn their needs then, working as a team, create plans to help members with those needs. Many of our members need help navigating the healthcare communities, community resources for social needs and maximizing their AHCCCS benefits. CHP is an integrated population management company, and our Medical Office Specialists work closely with our entire field team of Nurse Practitioners, Care Coordinators, Clinical Liaisons, and Behavioral Health Specialists to address the holistic needs of our members. We engage with members who are high cost, high risk and have low engagement with the health care community and our performance is based on how well we as a team address their complex holistic needs. We do not replace their community providers and clinics but help guide members on how to effectively use their community resources.

Responsibilities

  • Support our members with great customer service
  • Schedule appointments with our internal providers and external providers as needed
  • Make outbound calls to schedule and follow-up with transportation requests
  • Answer incoming calls from members as needed to support the team
  • Develop an understanding of the member’s social and physical needs
  • Build rapport and relationships with all members of the CHP team, including the members
  • Work as a team to meet member needs and plan for member success
