Casa Esperanza, Inc. is a bilingual/bicultural behavioral health treatment provider serving Boston and the Merrimack Valley. Position Summary:
The Medical Director will oversee the Medical Services at both our Tewksbury CSS and our Roxbury Outpatient program through Casa Esperanza’s Integrated Care Model, spending 10 hours per week at each site. The incumbent will oversee medication reconciliation process, supervise medical and nursing team, approve assessments and evaluations and provide consultation and supervision to on-site medical staff. They will prescribe medication as needed, and participate in quality assurance and utilization review activities. The Medical Director will lead the Multidisciplinary Team Meetings on both campuses.
REQUIREMENTS:
- Massachusetts Licensed Medical Doctor
- Board Certified MD
- Must be certified to prescribe buprenorphine and other medication assisted treatment for substance use
- Medicaid/Medicare enrolled provider
Knowledge/Skills/Abilities:
- Ensure patient confidentiality in accordance with all federal, state, and local laws and regulations
- Knowledge of best practices associated from the American Society of Addiction Medicine
- Knowledge of Medication Assisted Treatment for substance use disorders
- Knowledge of substance use disorders
- Knowledge and treatment of complex co-occurring, addictions, psychiatric and medical conditions
- Spanish Language fluency (preferred)
- Communication: Must have excellent oral, writing and listening skills
- Organizational skills: The ability to work independently, prioritize tasks, and create and coordinate schedules for supervision
- Interpersonal skills: Must have the ability to interface with participants as well as all levels of staff
- Computer skills: Must have experience with electronic health records and knowledge of Microsoft Office, Word and Excel
Job Duties and Responsibilities:
- Serve as the Medical Director of record for Casa Esperanza’s Behavioral Health programs
- Provide direct psychiatric services to clients
- Provide prescriptions for clients and complete medication reviews, as needed;
- Conduct and oversee clinical assessments to determine eligibility and appropriateness for MAT
- Providing feedback to the team about the appropriateness of MAT service for the individual based on their needs. Managing transitions between medications and managing titration.
- Provide medical consultation to Casa Esperanza’s behavioral health clinicians and oversight of the weekly Multidisciplinary Team meeting
- Participate in Risk assessment, Risk management, Crisis intervention
- Ensure responsibilities regarding Duty to Warn and Mandated Reporting are fulfilled
- Assess and document patient eligibility for disability benefits (SSI, DMH, USCIS, etc.)
- Approve nursing Assessments, Review COWS and CIWA exams for level of care appropriateness
- Conduct psychiatric assessments and evaluations for higher level of care psychiatric Section 12 determination in collaboration with Program Director and Nurse
- Triage emergency and non-emergency situations based on examination and prescribe medications as needed
- Work collaboratively with Nursing staff to provide medication reconciliation and prescriptions
- Ordering and interpreting diagnostic tests, and going over results with Nurse or client if needed for education and continuity of care
- Work with Clinicians and MDT to develop integrated treatment plans for clients prescribed buprenorphine products
- Work closely with clinical team during MDT meetings to offer and provide client medical insight as part of case presentations and feedback to treatment modalities.
- Works in collaboration with Pharmacy on medication refills and medication orders
- Works in collaboration with community providers including local Hospital and Clinics, to ensure referral to specialty care for medication, diagnostic and testing needs.
- Overseeing and supervising the multidisciplinary team in establishing integrated care management plans, including prescribing medications or other treatments.
- Educating the Multidisciplinary Team about medical conditions, psychiatric conditions and courses of action or treatment needed.
- Supervise nurse and offer training and coaching to medical team keeping record of supervision
- Provide on call support to Nurse and Program Director for medical emergencies, medication needs and medical incident reports as they arise.
- Ensures that addiction medicine services practice meets regulatory standards of each program
- Ensures that addiction medicine services care is appropriately documented in the medical record
- Participates in development of electronic record to ensure that clinical material is appropriately documented and privacy maintained.
- Maintain DATA waiver up-to-date
- Consult with staff and attend case conferences, as needed
- Provide off-site consultation, as needed
