```Duties```
As a Case Manager, you will be responsible for providing comprehensive case management services to patients. Your primary duties will include:
- Conducting patient assessments and developing individualized care plans
- Collaborating with healthcare providers to coordinate and manage patient care
- Monitoring patient progress and making necessary adjustments to the care plan
- Providing education and support to patients and their families
- Conducting diagnostic evaluations and coordinating appropriate treatment plans
- Utilizing your knowledge of physiology to assess patient needs and develop appropriate interventions
- Collaborating with behavioral health professionals to address mental health needs
- Implementing utilization management strategies to ensure efficient use of resources
- Assisting with discharge planning and ensuring a smooth transition of care
```Qualifications```
To be successful in this role, you should possess the following qualifications:
- Valid Registered Nurse (RN) license in the state of [Kentucky]
- Bachelor's degree in Nursing (BSN) preferred
- Proven experience in case management or a related field
- Strong knowledge of patient assessment techniques and diagnostic evaluation
- Familiarity with behavioral health principles and interventions
- Experience in utilization management and ensuring appropriate resource allocation
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced healthcare environment
