POSITION SUMMARY

Evaluate the needs of THP clients, identify the services needed, and create a plan to provide and monitor that care.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Essential functions include but are not limited to the following:

• Perform a case management needs assessment on clients in the recovery program to assist in barrier removal, increase recovery capital and improve self-sufficiency.
• Develop a service plan with measurable goals in conjunction with the client to address identified needs
• Maintain a case load of at least 25 clients with a minimum of 4 client contacts per month
• Maintain clear, concise documentation of services provided and goal progression in an EHR
• Maintain working knowledge of available community resources and processes for accessing resources
• Develop relationships with community partners and potential referral sources
• Monitor client progress through frequent meetings with client
• Assist clients in accessing additional services through referrals to outside agencies (ex. mental health, social security office, employment)
• Work in coordination with Peer Support Specialist and Clinicians to assist clients in meeting identified goals
• Perform periodic reassessment of clients, adjusting goals as needed based on client's changing needs
• Collaborate with departments within the Healing Place for client needs (ex. Detox services)
• Perform advocacy activities on behalf of clients as needed (ex. writing letters)
• Serve as liaison for clients involved in other systems such as legal and criminal justice
• Communicate with supervisor and team members on all issues related to clients
• Attend staff meetings and client case reviews as scheduled
• Maintain an up-to-date roster of current clients
• Other duties as assigned

QUALIFICATIONS / REQUIREMENTS

• Bachelor's Degree required
• Case management experience required
• Targeted Case Management training preferred
• Valid Driver's License
• Ability to work a flexible schedule
• Strong organization and time-management skills
• Willingness and ability to travel frequently in community and between facilities
• Must have or be able to learn and apply basic computer skills, including Microsoft products such as Excel, Word, and Outlook
• Must be able to achieve CPR certification within 3 months of hire
