Care Design NY is the largest Care Coordination Organization/Health Home (CCO/HH) in New York State. We provide person-centered comprehensive care management services for 27,000 individuals with intellectual and/or developmental disabilities (I/DD) in 30 New York State counties, so they may lead a quality life. We utilize a person-centered and holistic service planning approach to coordinate care and deliver integrated supports and services with individual, family and provider input. To learn more, please visit: http://caredesignny.org/careers\r\n\r\nResponsible for advocating for and with our members to ensure informed decision making, informed consent, and appropriate guardianship.\r\nResponsible for scheduling, leading and actively collaborating with our members and their interdisciplinary team to conduct meetings and assessments ensuring the development of a comprehensive, person-centered Life Plan that reflects the person’s needs and desired life goals.\r\nImplement, update, and monitor Life Plans and facilitate individualized Life Plan reviews and approval processes.\r\nEnsure integration of all needed and preferred supports and services (i.e., medical, behavioral, social, habilitation, dental, psychosocial, and community-based, and facility-based long-term supports and services, etc.).\r\nMaintain ongoing contact with the critical people in a member’s life.\r\nEnsure timely submission of all documentation (Life Plan, Progress notes, etc.)\r\nAssist our members with maintaining benefits such as Social Security, Supplemental Security Income, Medicaid and Medicare coverage, and Food Stamps.\r\nAssist members to resolve problems in living such as housing, utilities, the judicial system, and general safety.\r\nReport abuse or neglect immediately when observed or reported.\r\nA comprehensive job description may be provided during the interview process
