A renowned healthcare system in New York is searching for a Home Health RN for its territories east of Albany. In this role, you see 3-6 patients per day and provide compassionate one-on-one care. Responsibilities:

• Deliver direct and compassionate patient care in a home setting, utilizing the standard nursing practice, including assessment, planning, intervention, implementation, and evaluation.
• Develop and implement appropriate care plans based on the specific needs of the patients and families.
• Educate patients, caregivers, and families on how to appropriately manage care.
• Document treatment and progress toward desired outcomes.
• Administer medications to patients per MD orders.

Qualifications:

• Graduate from an accredited nursing program.
• Active RN license.
• 1 years of home health experience.

• Full-time, permanent position.
• Schedule; Monday-Friday, days
• Pay $37-41/hr., depending on experience, mileage reimbursement bonuses.
