A renowned healthcare system in New York is searching for a Home Health Admissions Registered Nurse for its Albany and Rensselaer territories. In this role, routinely complete Start of Care and Resumption of Care visits and head-to-toe assessments of patients. Responsibilities:

• Complete Start of Care, Resumption of Care, and occasionally routine visits based on needs.
• Head-to-toe assessments of patients and documenting all findings.
• Develop and implement appropriate care plans based on the specific needs of the patients and families.
• Communicate between doctors and other healthcare team members.
• Educate patients on health maintenance, prevention, infection control, and safety.

Qualifications:

• Graduate from an accredited nursing program.
• Active RN license.
• 1 years of home health experience.

Additional info:

• Full-time, permanent position.
• Schedule; Monday-Friday, 8a-4p
• Pay $37-41/hr., depending on experience, mileage reimbursement bonuses.
