The leading provider of healthcare services and senior living in Connecticut is searching for a Home Hospice Admissions Nurse for its Wallingford territory. In this role, you will be responsible for admitting eligible home hospice patients, providing nursing assessments, and developing care plans that optimize patients’ comfort and quality of life.

Responsibilities:

• Complete approximately eight admissions per week.
• Perform nursing assessments of potential hospice patients.
• Develop individualized care plans for patients and provide guidance and support to patients and their families.
• Collaborate with other healthcare professionals, including physicians, nursing assistants, social workers, and ministers.
• Help families cope with the upcoming loss.

Qualifications:

• A BSN is preferred.
• Active RN license.
• Home hospice experience is preferred.

Additional info:

• Full-time, permanent position.
• Monday-Friday, 8a-5p
• Compensation: $90,000-$110,000, depending on experience.
